Objectives To evaluate three-dimensional digital subtraction angiography (3D-DSA) in the diagnosis and endovascular treatment (EVT) of intracranial aneurysms, especially for aneurysm with wide neck. To determine whether EVT is feasible, to assess the possibility of achieving complete embolization of aneurysms, and to explore the main factors that effect achieving complete embolization of the aneurysm.Materials and Methods From Oct. 2004 to Oct. 2005, ninety-seven patients diagnosed as intracranial aneurysm were study with 2D-DSA and 3D-DSA. In which 88 cases complicated with subarachnoid hemorrhage confirmed by CT scan, 4 with mass effect of intracranial lesion, 2 patient with right paralysis and 3 patients with left side symptom. All patients were performed with anteroposterior, lateral DSA and rotational DSA. After rotational DSA, the data were transport to 3D-DSA workstation, where maximum intensity projection (MIP), shaded surface display (SSD) and volume rendering (VR) images were reconstructed within 5 minutes. The roles of 2D-DSA and 3D-DSA were evaluated in five aspects: 1. Detection of aneurysm; 2. Overall image quality; 3. Configuration of aneurysm; 4. Depiction of aneurysm neck; 5. Demonstration of aneurysm and adjacent artery. Data were collected and analysed and variance and multiple tests were conducted for 50 aneurysms treated by EVT, P< 0.05 was considered as statistically significant.
