Estimation Of Coronary Stenosis By Coronary Sinus Blood Flow Reserve Using Transthoracic Echocardiography

Objective This study sought to detect coronary sinus and its flow pattern and quantity coronary sinus flow and flow reserve for estimation the coronary stenosis by holdgrip exercise echocardiorgraphy and the reflow of coronary artery after percutaneous transluminal intervention(PCI).Methods 126 patients were accepted coronary angiography(CA) .According with the results of CA, 31 of them were diagnosed normal(A group),31 patients were left anterior descending(LAD) coronary stenosis(B group), 28 patients were LAD+LCA or RCA stenosis(C group), 21 patients were LCA and /or RCA stenosis(D group) ,16 patients were three branches stenosis(E group). The degree of stenosis was showed as Gensini index. The control group cases came from regular physical education. In every group, the diastolic coronary sinus peak flow velocity /index (DCSV/DCSI) and coronary sinus antegrade flow velocity/index(CSV/CSI)was measured before and after handgrip exercise echocardiography by HP SONOS 5500 at right heart inflow view. The coronary sinus velocity reserve(CSVR)and velocity index reserve(CSIR)was showed relatively as the ratio of CSV and CSI before and after handgrip exercise. The correlation between the CSVR/CSIR and the Gensini index was analyzed. After 1 week of PCI, the flow of coronary sinus were measured again in B and C group.Results The coronary sinus flow were success showed in all of cases. In B,C and E groups ,there were significant lower than control groups and A group about DCSV/DCSLCSV/CSI and CSVR/CSIR. There were no significant difference in D group and A group,but they have significant lower than control group. A negative correlation was found between Gensini index and CSVR/CSIR in B,C and E group. In B and C group patients, there 59 cases were successfully undergoing PCI.
