Purpose: To discuss the feasibility of controlled hypotension and effect of cerebral metabolism with combination of remifentanil and nitroglycerine. Methods: Thirty six patients scheduled for elective surgery were randomly divided into three groups: Group R, remifentanil - induced hypotension. Group N, remifentanil and nitroglycerine —induced hypotension. Group C, remifentanil - maintained normal arterial tension, target control infusion of propofol and isoflurane inhalation (1 —2%), adjust remifentanil and nitroglycerine infusion rate for controlled hypotension , record HR , BP , ECG and BIS etc , and radial arterial blood samples and jugular blood samples were taken synchronously for measuring blood gas and blood lactate , SOD and MDA levels . Results: In group R and N, the blood presses descend fast and stabilization, R group patient's heart rate was felled simultaneously, but inject atropine can change it. No rebounding response was observed. During controlled hypotension HR, ECG, BIS D(a-jv)O2, blood glucose, LD, SOD and MDA showed no significant change . Conclusions: Remifentanil and combination of remifentanil and nitroglycerine can be used for controlled hypotension and there is no significant effect in cerebral metabolism.
