OBJECTSThis study was designed to investigate the platelet activation status in different types of acute coronary syndrome(ACS), to detect the affects of the different antiplatelet drugs and their combinations(aspirin, clopidogrel and ticlopidine), to explore the clinical and laboratory features of platelet resistance and its probable mechanisms, and to create a noval screening parameter of platelet resistance.METHODSSixty-five patients with ACS were recruited and divided into 6 groups: group A (n=10) consists of ST elevation acute coronary syndrome(STE-ACS) patients receiving aspirin(ASA) with a load dose of 300mg and followed by 100mg/d, and clopidogrel(CPD) of 75 mg/d; group B (n=10) of non-ST elevation acute coronary syndrome (NSTE-ACS) patients receiving ASA with a load dose of 300mg and 100mg/d, and CPD of 75 mg/d; group C (n=10) of NSTE-ACS patients receiving ASA with a load dose of 300mg and 100mg/d, and ticlopidine(TCP) of 250 mg/d; group D (n=15) of NSTE-ACS patients receiving only ASA with a load dose of 300mg and 100mg/d; group E (n=10) of NSTE-ACS patients receiving only TCP of 250 mg/d; group N (n=10) of STE-ACS patients before receiving antiplatelet drugs.Platelet aggregation was induced in vitro by using arachidonic acid(AA), adenosine diphosphate (ADP) and multi-inductors(MI), and the antiplatelet effects of ASA, CPD and TCP, and their combinations were analysed pre- and post-incubation of ASA in vitro by measuring the percentage of platelet aggregation and the yield of thromxane B2 (TBX2) produced by platelet aggregation.
