Objective: To evaluate the therapeutic effect and safety of the carotid angioplasty and stenting (CAS) with a cerebral protection of devices initially.Methods: Eight cases (6 males,2 females, stegnosis from 50% to 94%, mean 68.7%,four cases <70%) with carotid artery stenosis were investigated.Cervical Doppler sonography,Transcranial Doppler(TCD), Digital subtraction DSA and NIHSS were performed in all cases.All patients were treated by means of endovascular stent under local anesthesia, 8 French femoral artery sheath was inserted after successfully Seldinger's puncture and 8F guiding was inserted in CCA near stenotic position,then cerebral protection of devices(SpiderRX 6.0mm) was placed in ICA about 4-5cm from stenotic position and released thoroughly.Self-expandable stent(Protege GPS) was threaded through stenotic position and released in ICA covering stenotic position.Antiplatelet drugs were given orally routinely after operation. Cervical Doppler sonography,Transcranial Doppler(TCD) and NIHSS were performed in all cases in three months,six months and twelve months postoperatively. Improvement degree of artery stenosis and change of cerebral haemodynamics were assessed.7 patients were followed up for 3.5-12 months(means 7.3 months),to observe complications of neural incidents,Stroke,TIA or death in the periprocedual and follow-up period,to evaluate carotid artery restenosis.Results:7 of 8 patients were treated CAS with Cerebral protection of devices successfully,one patient was quited for arteriospasm. One with stegnosis 94% underwent predilation,3 patients with residue stegnosis >30% underwent postdilation.
