Objectives: Acute lung injury/acute respiratory distress syndrome (ALI/ARDS) refers to acute progressive respiratory failure with stubborn hypoxemia induced by different predisposing factors except cardiogenic causes. ALI and ARDS have the same manifestations but different degrees in the changes of pathophysiology, ARDS is the most severe form of ALI.The causes of ALI/ARDS are very complex, more than 100 diseases associate with the incidence of them. In clinic, the causes include the ARDSp (primarily from pulmonary disease) and ARDSexp(primarily from extra pulmonary disease. Examples of ARDSp include pneumonia,aspiration and toxic fume inhalation, while systemic sepsis and nonthoracic trauma result in ARDSexp. As much as 30% of ALI/ARDS is associated with aspiration of gastric contents, and 40% with sepsis.
