Objective: To analyze the indications and effects of the neurosurgical and endovascular treatment of intracranial aneurysms in different type of intracranial aneurysms and in different situation of disease. Evaluating the advantages and disadvantages of the two types of treatment.Methods: All the 92 patients treated from June 2004 to December 2005 in the second hospital of ShanDong University were assigned to either neurosurgical or endovascular treatment,the number is 44 and 48 respectively. 37 males and 55 females were included, the age averaged 49.6 years distributed from 13 to 74 years. To analyze the indication according to the site, mordality, size of aneurysms and patient states on admission.To study the treatment effect on the basis of GOS grade. Results: The neurosurgical and endovascular management were both effective in most aneurysms.GOS grade 4 and 5 are considered the favorable effects. The favorable rate was 77.3% in surgical group and 91.7% in endovascular group,the mortality was 4.5% and 4.2% respectively.Conclusions: According to the results of this research, the absolute indications for determining method of treating intracranial aneurysms are encountered infrequently.However, neurosurgical method is unique for treating Hunt-Hess V grade patients with large hematoma in the cavity of skull,it should be considered when the giant aneurysms show mass effect,the diameter of parent artery is very small and the operated vessels are in a state of svere spasm. We also conclude that endovascular therapy is dominant in intracavenous carotid aneurysms,posterior-circulation aneurysms and multiple intracranial aneurysms.
