OBJECTIVE To find a scatheless method of early detection and routine screening of bladder cancer, by detection of Survivin, an inhibitor of apoptosis, in urine specimens and tumor tissues of transitional cell carcinoma of bladder. Quantitate expression of Survivin in urine and tumor tissue and to investigate its dependability with grading and staging of tumor pathology and prognosis. METHODS Take 200ml fresh midpiece urine sample from 53 patients of transitional cell carcinoma of bladder, 20 patients of other urologic disease, and 10 healthy volunteers. Take tumor tissue from 53 patients with transitional cell carcinoma of the bladder(same as the above) postoperatively and 5 normal mucous membrane of bladder. Detect the expression of Survivin by nested RT-PCR, voided urine cytology and biopsy through cystoscope simultaneously. Quantitate the Survivin expression by real-time quantitive PCR. RESULTS Expression of Survivin is detected in all 53 patients of bladder transitional cell carcinoma by nested RT-PCR, the sensitivity and specificity of expression of Survivin in all urine samples arel00%, 90% respectively. Sensitivity and specificity of voided urine cytology are 37.7%, 100% respectively. The sensitivity and specificity of biopsy through cystoscope are both 100%. The sensitivity compared urine sample and cancer tissue with voided urine cytology, (p<0.01), has significant deviation, in the same with cystoscope examination.While, specificity are 90%, 100% and 100%, respectively, (p>0.05), without difference.
