Objective: To investigate the prevalence of Cervical cancer and Cervical intraepithelial neoplasia(CIN) of women in the rural province of Shanxi as well as positive rate of high risk human papillomaviruses (HPV). To identify attitudes about and barriers to receive an HPV vaccine.Methods: A population-based cervical cancer screening and a questionnaire assessing knowledge about HPV vaccine were done in women with age between 15~54 in Xiangyuan, Shanxi Province. There were two steps: first, after consent all eligible sexual women underwent a self-sample for HPV testing by the instructing of health care professional, called'Self-test for HPV'; Second, Collection of cervical samples will be performed by a health care professional for HPV testing, called'Direct HPV test', and exfoliated cervical samples were also collected for Liquid-based cytology. VIA, and coloscopy examination and biopsy were provided later if any abnormal lesion were found. Eventually, the golden standard to evaluate all screening method is pathology results. If nonaccess women choose to participate in this research, vaginal sample was collected for HPV testing by a health care professional. Each subject was administered an epidemiology questionnaire as well as the acknowledgement of HPV vaccine.Results: Eligible 888 women went through above examination procedure finally. There were 1 cervical cancer, 13 cases CIN III, 8 cases CIN II, 41 cases CIN I, the prevalence of CIN was 7.2%, HPV DNA (hc2) detection rate in the population was 19.4%.The average number of years of education was 7.2, Mean family income of participants was 3511.26 Yuan per year. Questionnaires were completed by all participants, of whom 5.9% had heard of HPV, but even among those who had heard of HPV, knowledge was generalpoor, and only 4.8% were aware of the link with cervical cancer; 85.9% were unclear of the risk of infection by HPV. Acknowledge of HPV increased with age and education(p<0.05); But there was no association between Acknowledge of HPV and experience of an abnormal smear and had family history of cervical cancer(p>0.05). After simple informed consent, 96.5% of women were willing to accept HPV vaccine; And 49.9% of women were more likely to receive the commends by health care professionals than members of family and friends to vaccination.Conclusions:1. The prevalence of CIN OF women in shanxi province was higher.2. Cost, security and efficacy, women's awareness of HPV infection and recommendation by health care professionals were barriers to vaccination.3. Public education is urgently needed. Successful implementation of a vaccination program will require a coordinated effort of Promotion and organization of government, appropriate insurance coverage and health care establishment.
