Objective:To explore the effect of ABO- incompatibility on clinical characteristics in allogeneic hematopoietic stem cell transplantation (allo-HSCT). Methods: We retrospectively analyzed the clinical courses of seventy two recipients (1985.4~2006.4) of allo-HSCT from ABO-incompatible donors(32 cases were major ,33 cases were minor and 7 cases were bi-direction ABO-incompatibility),Eight three ABO-compatible recipients were used as control group. Results: In major incompatible group, the time of erythrocyte recovery after allogeneic hematopoietic stem cell transplantation (allo-HSCT) was delayed and more RBC transfusions were required as compared with ABO compatible group. The incidence of acute graft versus host disease (aGVHD), cytomegalovirus(CMV) infection and survival had no significant difference between ABO-incompatible and ABO-compatible recipients from allo-HSCT. Conclusion: ABO-incompatibility delayed erythrocyte recovery occur and has no adverse effect on the outcome on the allo-Hsct, but in major incompatible group, some patients got pure red cell aplasia(PRCA).
