AimTo evaluate the approach of treatment of severe acute pancreatitis of different types and stages.MethodsTo collect the materials of 76 patients whom was checked in our hospital from Jan 2001 to Dec 2006 with severe acute pancreatitis(SAP), including different origin, treatment and result. They are separate into 4 groups, expectant treat-group, traditional operation-group, ERCP-group, and LC+ERCP group. Inorder to see which treatment is the most effective and safe.ResultsOf all the 76 patients, 53 were induced by bileduct obstruction, while 3 were alcohol, 7 were hyperlipemia and 13 were unknown. Through different treatment, 56 patients were healed, 20 were dead or leave the hospital. In these patients, emergency or traditional operation-group have the highest death rate, while the ERCP group is the most appropriate and effective.ConclusionsThe first motivation of SAP is still biliary tract disease, with a rate of 69. 7% in the 76 patients. As a result, to get through the obstruction or other disease of biliary duct is the key point to heal SAP patients. Further more, we see 7 individual in 76 experience hyperlipemia with severe acute pancreatitis. The rate is 9.2%, with expectant treatment and control hyperlipemia et al. The result is all good.Expectant treatment is very important in SAP, effective and appropriate management can hold the station prominently. The indication of operation need to choose carefully, because of its high death rate with huge attack at the operation patient getting through. ERCP with LC is a new choice for SAP. Its safety and efficacy is outstanding, but advantages and disadvantages is still on doubt.
