Semen Quality Changes In The Patients With Varicocele And Infertile Patients After Varicoceletomy

The varicocele is a familiar disease that leads to man's infertility , its'incidence is about 10%～15%. Many facters lead to infertility, but the definite reason is still unknown. The accepted seasons are temperature rise, tiny cycle obstruction, metabolite accumulation, oxidation damnification and immunity inducement. To evaluate the semen quality and sperm morphology changes in the patients with varicocele and infertile patients before and after varicoceletomy. Semen from 121 patients with varicocele were studied by the method described by WHO and those of 23 normal male were taken as the control. A comparison of their semen parameters both before and after varicoceletomy had been made in the 21 infertile patients.A significantly reduction of sperm density,A+B grade sperm percentage, sperm viability, number of effective sperm, motility sperm and sperm motility index were found in patients with varicocele comparing with those of the control(P<0.01). But, sperm density,A+B grade sperm percentage and sperm viability in the general sperm detect can not estimate the damage of the patients's eugenesis in time and effectly. This observation shows that 39.7% ofthe varicocele patients'sperm density,A+B grade sperm percentage, sperm viability motility sperm and sperm motility index were normal, if they were only taken in the general sperm detect. But if they were taken in the morphology sperm detect, only 1.7% of the varicocele patients'sperm morphology were normal. It is different from the results of different former observer; maybe the methods and experiment standard used are different. An increase of small oval head, tapering head and amorphous head sperm were found in patients with varicocele comparing with those of the control(P<0.01). Although there were some differences of the reports on the abnormal morphology sperm between different scholars, the head abnormal sperm were most. The mechanism that cause sperm abnormality is still unknown, it may relate to the unconventionality of sperm nuclear maturation and death. The varicoceletomy wipe out the varicose veins'gore, testicle anoxia, poison backflowing, lower the varicose veins'blood pressure, resume the enzyme activity, and correct the foul-up of energy metabolizability, so it can resume the testicle function. An increase of sperm density,A+B grade sperm percentage, sperm viability,number of effective sperm, motility sperm , sperm motility index and normal morphology sperm were found in patients with varicocele comparing with those of the control(P<0.01,P<0.05,P<0.001). After varicoceletomy, the amorphous head sperm and round head sperm were reduced while other abnormal morphology sperm were not changed obviously. In the 21 infertile patients, 5 were fertile ,and other 16 patients were still infertile. It may relate to a short follow-up.In conclusion, the varicocele can reduce the semen quality ,and sperm morphologic assessment with WHO criteria provides a sensitive measurement of sperm damage in the patients with varicocele. And the varicoceletomy can improve the semen quality.
