This research includes two parts:Part one: A clinical study of 125I brachytherapy combined with total androgen blockade for prostate cancer.Objective: To investigat the clinical effects of 125I implantation combined ewith total androgen blockade(TAB) in the treatment of adenocarcinama of the prostate. Methods: To analyze retrospectively 9 patients with stage C and stage D1 prostate cancer. All patients accepted the combined treatment of 125I seeds intraoperative implantation, bitateral orchidectomy, and Flutaminde (250 mg every 8 hours) in this hospital. Results: Follow-up periods ranged from 12 to 36 months. After 6 months postoperatively, PSA value decreased from 72.1±20.4ng/ml preoperatively to 3.6±0.6 ng/ml (PSA decreased to <1 ng/mL in 6 patients), prostate volume dropped to 37.73±10.3cm3, and IPSS score dropped from 30.2±2.1 preoperatively to 11.5±5.3(P<0.05). After 12 months postoperatively, PSA decreased to 1.8±1.3ng/ml in 8 patients. In 1 patient with stage D prostate cancer PSA increased to 30.9 ng/ml. Conclusion: the procedure had the advantage of both radioactive seeds interstitial brachytherapy and total androgen blockade and was safe and effective with fewer complications. . It is believed that the combined use of 125I implantation and total androgen blockade be one of the ideal treatment for prostate cancer.Part two: An experimental study radiation injury on rabbits urethra induced by interstitial 125I seeds implantingObjective: to assess radiation injury on rabits urethra induced by interstitial 125I seeds implanting. Methods: A total of 10 rabbits were randomly divided into experimental and control group with body weight 2.0~3.2kg. 125I seeds was implanted near urethra under anaesthesia, and the irradiation doses by 125I seeds were 0.4 mCi, 0.8 mCi and 1.2 mCi, respectively. then we detected the pathology of urethra and observed the morphologic change with H.E. at 4 weeks after seeds implanting, urethral X rays was performed in experimental group. Results: At the 4th week postoperative,. histopathological and ultrastructural changes of the urethral tissue were not obvious as compared to the contral group. Conclusion: In the rabbit model, there is an obvious dose response relationship for in the 125I seeds.
