Plasma Adiponectin Level In Obesity Patients With Essential Hypertension And Its Affecting Factors

Objective:To study the correlation between plasma adiponectin and blood pressure(BP), blood mass index(BMI), insulin resistance (IR) and high-density lipoprotein cholesterol (HDL-C) by detecting plasma asiponectin levels in normotensive (NT) and essential hypertensive (EHP) male patients with or without obesity (BMI≥25kg/m2).Methods:It is a case-control study. 124 patients were enrolled in the study. The EHP group consisted 54 patients, among them 31 patients with BMI≥25kg/m2 obesityed. The NT group consisted 70 patients with 30 obesityed. Fasting plasma adiponectin level (by radioimmunoassay), fasting insulin (by electorchemiluminescence immunoassay), fasting blood glucose (FBG), BMI, BP, waist circumference (WC), total cholesterol (TC) , triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C) ,blood uricemia acid (UA) were measured in each patient and the insulin resistance index (IRI) was calculated according to the typical HOMA mode l(HOMA-IR). The differences on each parameter were compared among subgroups and groups. The correlations between adiponectin and other parameters were investigated by multiple linear stepwise regression analysis.Results: 124 samples were involved in the result analysis. (1) The adiponectin level in EHP group (7.48±3.4μg/ml) were significantly lower than that in NT group (10.57±4.73μg/ml, p<0.001). (2) The plasma adiponectin level was lower in EHP group with obesity than in the other three subgroups (p<0.05). Patients with obesity has lower adiponectin level than non-obesity ones (p<0.05). (3) No significant difference in plasma adiponectin between obesity NT patients and non-obesity EHP ones(p=0.069). (4) The differences on all the parameters except UA (p=0.32),FBG (p=0.14) and age (p=0.05) was extremely significant(p<0.05) between EHP and NT groups. Multiple linear related analysis showed significantly negatively correlations between plasma adiponectin and WC, BMI, BP, TG, FINS, HOMA-IR and positively correlation between plasma adiponectin and HDL-C levels. BP, WC, BMI, HOMA-IR were the most significant factors that affecting the level of plasma adiponectin.Conclusion: (1) The plasma adiponectin levels were lower in patients with EHP, and even lower in patients with obesity and EHP. (2) Compared to non-obesity patients, obesity patients with or without EHP showed lower plasma adiponectin level. (3) There was an independent correlation between the levels of plasma adiponectin and blood pressure, which may imply adiponectin might be a useful indicator for obesity patients to develop hypertension.
