| Objecttive To analyze the main causes of periprosthetic femur fractures after total hip arthroplasty retrospectively and to explore the best operative methods and precautions for fractures.Methods 15 cases treated for periprosthetic femoral fracture after total hip arthroplasty were subjected to a retrospective follow-up study from February 2002 toDecember 2006.There were 6 men and 9 women, the mean age was 65 years (range, 48 to 82 years). According to the classification of Vancouver, there were 2 A type fractures, 3 B1 type,6 B2 type,1 B3 type, 3 C type. Two fractures were treated by nonoperative methods and other thirteen by operative methods, seven fractures treated by open reduction and interal fixtion.. There were four revisions using long uncemented stem supplemented with autograft, and one with the cortical allograft struts.Results One had been died because of the medical complications , by skin traction for forty-four days. Through eight to forty-eight months follow-up (the mean follow-up period was 24.5 months), 14 fractures were united, the meanHarris score was 89. 5, and the Harris score did not change significantly in those who received fracture internal fixation merely without femur stem reversion, and increased significantly in those who received fixation and femur stem reversion at the same time.Conclusions To evaluate carefully the patient's condition overall and accord to Vancouver classification , it can get a satisfactory curative effectin . |
