Background: The eradication rate of the standard triple therapy for treating Helicobacter pylori(H. pylori) infection is declining. Whether the quadruple therapy containing bismuth could be more effective than the standard triple therapy for the first—line eradication?Aims: To observe the efficacy of H. pylori eradication with 7-day rabeprazole-based triple therapy versus quadruple therapy containing bismuth for the treatment of H. pylori infection.Methods: A total of 65 consecutive patients with non—ulcer dyspepsia who were H. pylori positive proven by both rapid urease test and histology were included and randomly assigned to 7-day raheprazole-based triple therapy or quadruple therapy. 32 patients were administered with triple therapy including rabeprazole(R)10 mg bid, clarithromycin(C)500 mg bid and amoxicillin(A) 1.0 g bid. 32 patients received quadruple therapy combining the above triple regimen with colloidal bismuth subcitrate(B) 220 mg bid. The eradication rates were evaluated by14C-urea breath test at least 4 weeks after completion of treatment.Results: A total of 60 patients(30 in RCA gmup. 30 in RBCA group)completed the study. The H. pylori eradication rates by intention—to—treat analysis in RCA and RBCA group were 7 1.9%and 75.8%, respectively,by per protocol analysis were 76.7%and 83.3%. There was no significant difference statistically of the eradication rates between the two groups (P>0.05). Except tow patients in RBCA group could not complete a course of treatment because of moderate to severe dizziness and abdominal pain, respectively, other patients of the two groups had similar short—lasting and self-limited side effects including metallic taste. diarrhoea。 abdominal pain. dizziness and nausea.Conclusions: It is concluded that both 7-day rabeprazole—based triple and quadruple therapy are effective therapeutic regimens. RBAC regimen has not shown significant benefit on H. pylori eradication over the triple therapy.
