Background and objectiveWith requiring advance of operative analgesia of patients and the development of anesthesiology, pharmacy , and medical model transforming , intraoperative and postoperative analgesia attached more and more importance in patients and operators. General anesthesia can achieve anodynia in operating, but high price , many side effects as influcing breath and vascular , cause obviouse pressure and many postoperative questions .So a lot of operations should be used local anesthesia as far as possible . However, local anesthesia has few questions , for instance , operation can not achieve total anodynia , patients have mental stress additionally so that operations cannot be completed smoothly ,and even influence prospective results . The past years preoperative analgesia as Sauteralgyl and fentanyl etc. are administrated by anesthesia drugs .Because of its usage being limited , common ophthalmology operations are not suitful. Using the powerful analgesia drugs whose side effect can be ignored ,used conveniently is the common goal for patients and operators.Tramadol or Tramal is a kind of hadro-petency pain-killers which was used in recent years . Its analgesia effect is corresponds to meperidine's , and only as one-tenth as morphinic's . It cannot influence cardiovascular system function and inhibit breath and low addiction ,low dependence , low tolerance in therapeutic dose . Consequently, TR has being applied generally and attached importance in recent years . This drug is commonly used to keep the postoperative and cancer patients from pain.Some department as stamotology using THI before local anesthesia operations can relief inoperative and postoperative pain . THI is commom used in ophthalmology postoperative pain . However , until recently there is no report on injecting THI before operation to relieve during and after operation.pain . This thesis object is to find the optical injecting time before operation and the drug effective which is during and after operation.Methods1. Cases selection and test methods 113 patients were selected from the department of ophthalmology of the First Affiliated Hospital of Zhengzhou University (59 men and 54 women ) who were one eye disease of complicated ,long time and / or dragging muscles more times during operation ( age 15-78 years , mean age 41.37 years ). They were divided into four group randomly , preoperative 10 minutes , 20 minutes, 30 minutes group ( experiment group) was intramuscular injected same dose , same specification , same factory productive tramadol hydrochloride injection 100 mg and un- intramuscular injecting group, observing eyes' ache during local anesthesia ; using 1 milliliter muticate pinhead to touch eyelids conjunctiva cornea and using 1-0 or 3-0 suture silk to drag muscles properly query patients' ache condition and patients' chief complaint during operation ( 0.5h,1h,1.5h,2h,2.5h ) ; inquesting ache condition when moving eyeball or not after operation ( 1h,2h,4h,6h ). Working out pain grade and comparing with them according to Four grade grouping.2. Statistical analysis Taking questionnaire quantization wholly , all statistical analysis were performed with SPSS 13.0 software . Deploying LSD multiple comparion and matched-pairs t-test to analyze during local anesthesia; deploying two-sample u - test for independent samples to analyze for two groups' mean ache grade during local anesthesia and operation and after operation .Results1. The Fisher's test showed that F=1 .96 , P>0.05; preoperative 10 minutes group, preoperative 20 and 30 minutes group had significant , P<0.05; preoperative 10 minutes group and the control group had no significant, P>0.05, preoperative 20 and 30 minutes group had no significant, P>0.05.2.The experiment group and control group during anesthesia had significant , u=4.565, P<0.001; The experiment group and control group during operation had significant , u=8.108, P<0.001; The experiment group and control group after operation had significant, w=2.073, P<0.05.ConclusionThis study certified that assistance analgesia was effective in ophthalmology local anesthesia preoperative intramuscular injection with Tramadol Hydrochloride Injection during and after operation., and relieved patients' nevious and fearful moods; entrying into force of tramadol hydrochloride injection was about twenty minutes after intramuscular injection and its biggest effective time was 1-1.5h and its sustaining time was 3-6h .
