Objective Explore to improve a 2nd metatarsal-to-hand transfer procedure for better metacarpo-phalangeal joint movement and looking by selective subcondral metatarsal joint bone cut. Method 1. 30 adult dry bone specimens, measure their metatarsal joint bone surface. 2. 20 adult foot specimens, detailed anatomy on the first toe dorsal artery. 3. A series of 6 cases with a toe-to-hand transfer procedure in practice and long-term follow-up examination. Result 1. adult dry bone specimens measurement: take metacarpo-phalangeal joint Lig. collaterale as the turning center, the turning radius from toe dorsal to base increase to 3.0±1.06mm, available for selective subcondral metatarsal joint bone cut. 2. anatomy type on the first metatarsal bone dorsal artery in consistent with Gu Yu-dong's type, and reach the metatarsal about 5.0mm from toe metatarsal joint. 3. the six cases, passive range of motion in extension-flexion direction are 85°maximally and 65°minimally, average 75°. Active range of motion 75°maximally and 65°minimally, average 75°. In X-ray films taken, the reconstructed digits are shown to be normal with no sign of degenerative changes.Conclusion a toe-to-hand transfer procedure for better metacarpo-phalangeal joint movement and looking by selective subcondral metatarsal joint bone cut is effective for better extension-flexion, available in anatomy, is an improved technique for better toe-hand reconstruction.
