Objective To evaluate the delineation of hypopharyngeal carcinoma and surrounding anatomic structures in CT scan under quiet breathing and modified Valsalva maneuver and to investigate the diagnostic value of modified Valsalva maneuver to hypopharyngeal carcinoma. Material and Methods Twenty-two patients with clinically suspected hypopharyngeal carcinoma were examined with spiral CT scan under both quiet breathing and modified Valsalva maneuver. The delineation and staging of the tumor with the two methods were analyzed and the comparison of CT and postoperative pathology was conducted. Results Modified Valsalva maneuver CT was superior to quiet breathing CT in the delineation of the tumor (n=15), pyriform sinus (n=18), atrium glottidis (n=10) and retropharyngeal wall (n=12) (P<0.05), postcricoid area (n=5) (P>0.05). Modified Valsalva maneuver CT was inferior to quiet breathing CT in the delineation of the ventricle of larynx (n=10) (P<0.05). The accuracy of the former was 90% and that of the latter was 64% in the tumor (T) staging respectively (P<0.05). Conclusion Modified Valsalva maneuver CT was superior in the delineation of the tumor, pyriform sinus, atrium glottidis, postcricoid area and retropharyngeal wall, inferior in the delineation of the ventricle of larynx. The T stages determined with modified Valsalva maneuver CT were better than that determined with quiet breathing CT.
