Object: to investigate the effects of the hamornic scalpel on Recurrent laryngeal nerve(RLN) in Rabbits and study margin of safety in which harmonic scalpel can be used. Methods: (1) Forty-five New Zealand rabbits were randonmly divided into nine groups(the application time in which harmonic scalpel directly touched the RLN was 1s, 3s, 5s; the application time in which HS was used at 1mm from the RLN was 1s, 3s, 5s; the application time in which HS was used at 2mm from the RLN was 1s, 3s, 5s).the parameters of the compound muscle action potential were obtained before-and-after experiment using the EMG instrument. (2) the specimen was removed after experiment,we could see the width of lateral thermal damage from the point of Harmonic scalpel application to the margins of unchanged nearby tissue by Masson staining. Result: (1) the significant difference of the parameters of the compound muscle action potential such as the minimal stimulus current instensity threshold(MSCIT) of evoked electromygrapy(EMG),the Best Stimulus Current Intensity Threshold(BSCIT), the onset latency, the peak latency and amplify was observed in the groups(the application time in which HS was directly touched the RLN was 1s, 3s, 5s; the application time in which HS is used at 1mm from the RLN was 3s, 5s; the application time in which HS is used at 2mm from the RLN was 5s). the significant difference of the peak latency of CMAP was observed in the group(the application time in which HS is used at 1mm from the RLN was 1s). No significant difference was observed in groups(the application time in which HS is used at 2mm from the RLN was 1s, 3s). (2): Histological Damage to the Circumference of the Rabbit RLN by us of HS:①The application time was 1s: <1mm.②The application time was 3s:1—2mm.③The application time was 1s:≥2mm. Conclusion: If we can identify the RLN in the laparoscopic view, HS is only used at more than 2mm from the recurrent nerve and for less than 3 seconds at level 3...
