Objective: To measure intraocular pressure (IOP) and axis oculi onchoriodal detachment associated with retinal detachment by differentopertations preoperatively and postoperatively. To approach the change ofIOP and axis oculi on choriodal detachment associated with retinaldetachment eyes whose retinas were successfully replaced, and to analyzethe relationship between the IOP,axis oculi and the course of disease.Method: 30 patients (30 eyes) with choriodal detachment associatedwith retinal detachment and whose retina were successful replaced in thefirst operation were collected from July, 2005 to July, 2006 in ourhospital, male 13,female 17. Grouping:①According to operationtechnique, the 30 patients were divided 2 groups: routine operation ofretinal replacement(operation of scleral encircling and buckling) group;vitreous body's operation of retinal replacement (operation of vitrectomyand scleral encircling and buckling) group.②According to the course ofdisease before treatments, the 30 patients were divided 3 groups: 1 month,2 months and 3 months. IOP and axis oculi were measured withGoldmann tonometer and A scan at the moment of diagnosis,1 daypreoperatively with the treatment of glucocorticoid and 1 week, 1,3,6months postoperatively.Result: 1. At the moment of diagnosis, the mean IOP of all patients'othereyes was (13.80±2.63) mmHg, and the eyes with disease was(5.00±3.47) mmHg, there was significant (P<0.01) between them. After7~10 days' glucocorticoid treatment, the IOP of disease eyes werehigher than before with significant (P<0.01). At 1 week and 1,3,6months postoperatively, the mean IOP of all patients' disease eyes weremuch higher than the IOP at 1 day preoperatively (P<0.01); the IOP at 1month postoperatively was lower than the IOP at 1 week postoperatively(P<0.05); among 1,3 and 6 months, there were not statisticallysignificant (P>0.05)2. At the moment of diagnosis, the mean axis oculi of allpatients'other eyes was (27.36±2.41)mm mmHg, and the eyes withdisease was (25.26±2.29) mm. After 7~10 days' glucocorticoidtreatment, the disease eyes'mean axis oculi was (25.26±2.02) mm, therewas not significant (P<0.01) relative to before. In the operation of scleralencircling and buckling group, the mean axis oculi of disease eyes at 1week,1 month,3 months and 6 months postoperatively were much longerthan 1 day preoperatively (P<0.01), but it got shorter from 1 weekpostoperatively to 6 months postoperatively, there was significant(P<0.01). In the operation of vitrectomy group, the mean axis oculi ofdisease eyes at 1 week,1 month,3 months and 6 months after theoperation of silicone oil removement were much longer than 1 day before operation of retinal replacement (P<0.01). there was not significant(P>0.05) from 1 week after silicone oil removement to 6 months aftersilicone oil removement.3. there was positive linear correlation between the IOP's differenceof two eyes in patients and the disease course before treatment (r=0.833,P<0.05)Conclusions:1. To choroidal detachment associated with retinal detachment, thedisease eye' IOP got low, axis oculi became short.2. IOP would decrease along with the course of diseases beforetreatment.3. Using glucocorticoid before surgery could improve state of illness,increased IOP.4. After successful operation the IOP increased and and axis oculiextended obviously. IOP decreased during 1 month postoperatively, andgot steady after 1 month of operations. Axis oculi had little change from 1week to 6 months after the operation of silicone oil removement, and axisoculi shortened during 6 months after the operation of scleral encirclingand buckling.
