Objective To investigate the expression of heme oxygenase-1(HO-1)and heme oxygenase-2(HO-2)in villi from patients with spontaneous abortion and the relation to apoptosis.Methods Forty patients with spontaneous abortion(20 threatened abortion cases and 20 inevitable abortion cases)and 30 normal early pregnancy were investigated.The expression of HOs mRNA and protein was determined with in situ hybridization and immunohistochemistry respectively.The villi trophoblastic cell apoptosis was examined by TUNEL.Results(1)The IOD values of HO-1 mRNA in normal pregnancy group and spontaneous abortion group were(11.41±1.98)and(10.07±1.95);the IOD values of HO-1 protein in normal pregnancy group and spontaneous abortion group were(10.67±1.65)and(9.26±2.15)(P<0.01).The IOD values of HO-1 mRNA in threatened abortion group and inevitable abortion group were(10.78±1.83)and(9.36±1.86),the IOD values of HO-1 protein were(10.07±2.27) and(8.45±1.71)(P<0.02).(2)The IOD values of HO-2 mRNA in normal pregnancy group and spontaneous abortion group were(11.56±2.13)and(10.44±2.03);the IOD values of HO-2 protein in normal pregnancy group and spontaneous group were(10.68±2.04)and(9.50±1.94)(P<0.05).The IOD values of HO-2 mRNA in threatened abortion group and inevitable abortion group were(11.13±1.71)and(9.74±2.13),the IOD values of HO-2 protein were (10.35±1.99)and(8.65±1.51)(P<0.05).(3)The villi trophoblastic cell apoptosis index(AI) in normal pregnancy group(13.24%±4.78%)was significantly reduced than in spontaneous abortion group(35.49%±9.71%)(p<0.001);the decrease was even more obvious in threatened abortion group(29.03%±7.83%)than in inevitable abortion group(41.96%±6.64%)(P<0.001).(4)There was negative correlation between the expression of HO-1 mRNA and protein and villi trophoblastic cell AI in normal pregnancy(r=-0.692 and -0.717,P<0.01),as well as in spontaneous abortion(r=-0.743 and -0.762,P<0.01),in threatened abortion group(r=-0.703 and -0.725,P<0.01),and in inevitable abortion group(r=-0.745 and -0.769,P<0.01);there is no correlation between the expression of HO-2 and villi trophoblastic cell apoptosis in this study. Conclusion The study show s that the decrease of heme oxygenase-1 and heine oxygenase-2 in villi takes a important role in incidence of spontaneous abortion,and abnormal villi trophoblastic cell apoptosis is related to decrease expression of HO-1.
