Objective: Ameloblastoma is the most frequently odontogenic epithelial tumor in oral and maxillofacial region. It mainly occurs in the adult and growed up slowly. But it is easy to attack partially and easy to recur after surgery. As everyone knows that the biology behavior is related with histology classification, follicular pattern and plexiform pattern are the main types in ameloblastoma. EGFR, Ki-67 and AgNOR were studied in ameloblastoma in order to discusse the proliferation activity and provide the laboratory data for the clinic prognosis of ameloblastoma.Methods: Seventy-one cases of ameloblastoma were collected from the second affiliated hospital of Hebei medical university from Jan.1985 to Dec.2006. Thirty-seven cases were male, and thirty-four cases were female, the ages ranged from 11 to 75 years old, the average age was 36.13 years old. Sixty cases were occured in the mandible, and 11 cases were occured in the maxilla. Thirty-nine cases were follicular pattern, and thirty-two cases were plexiform pattern. By the standard of histology classification in ameloblastoma which published in 1992, the specimens were stained with hematoxylin-erosion (HE), by this way, the dignosis was confirmed. The expression of EGFR, Ki-67 and AgNOR were detected by different method. Statistical analysis was performed with SPSS 10.0 software, and used Chi-square and T-test was used too. Statistically significant level was considered as "alpha equals 0.05".Results: 1) The expression of EGFR in ameloblastoma of the two histology types: EGFR was mainly observed in the cytoplasm and on the cell membrane of tumor cells. The positive rate of EGFR was 97.44% (39/39) in follicular pattern, and EGFR was mainly positive expressed in scattered stellate central cells in ameloblastoma of follicular pattern; it was 68.88%(22/32) in plexiform pattern, and EGFR was positive expressed in peripheral epithelium cells in ameloblastoma of follicular pattern. There was significant difference between two types( X2 = 11.04, P < 0.005 ).2) The expression of Ki-67 in ameloblastoma between the two histology types: It was observed in the cell nucleus and nucleolus. The positive rate of Ki-67 was 71.79% (28/39) in follicular pattern, and Ki-67 was mainly positive expressed in scattered satellite central cells in ameloblastoma of follicular pattern; it was 56.25% (18/32) in plexiform pattern, and and EGFR was mainly positive expressed in ameloblastoma of plexiform pattern。There was significant difference between two types (X2 = 34.14, P < 0.005). The proliferating indice of Ki-67 was significantly in follicular pattern( 4.48%±1.00% ) higher than that in plexiform pattern( 3.17%±0.78%, P = 0.0433 ).3) The expression of AgNOR in ameloblastoma of the two histology types: AgNOR was mainly observed in the nucleolus, and it was brown. There was significant difference between two types (X2 = 34.14, P < 0.005). The indice of AgNOR in follicular pattern was significantly (2.14±0.88) higher than that in plexiform pattern (1.62±0.74, P = 0.008).4) The relationship among EGFR and Ki-67 and AgNOR: The indices between EGFR and Ki-67 showed a significant correlation (r = 0.667, P < 0.001). The indices between EGFR and AgNOR showed a significant correlation (r = 0.468, P < 0.001). The indices between Ki-67 and AgNOR showed a significant correlation (r = 0.667, P < 0.001).Conclusion: 1) The expressions of EGFR and Ki-67 and AgNOR were obvious higher in follicular pattern than that in plexiform pattern,it pointed out that follicular pattern has the more proliferating activity than plexiform pattern. 2) EGFR was mainly discovered in scattered stellate central cells of follicular pattern, it pointed out that it has the more proliferating activity in scattered stellate central cells. It was mainly discovered in peripheral epithelium cells of plexiform pattern, it pointed out that it has the more proliferating activity in peripheral epithelium cells. 3) There was obvious related correlation among EGFR and Ki-67 and AgNOR; they might be involved in tumorigenosis and progression of follicular pattern and plexiform pattern ameloblastoma.
