Effects Of Total Spaonins Of Panax Notoginseng On The Compressed Peripheral Nerves

Objective: In order to observe the mechnicisms and effects of total spaonins of panax notoginseng on the compressed peripheral nerves. Accordingly,it provide the theoretical foundation for more reasonable cure of the compressed peripheral nerves .Methods: A chronic nerve compression animal model was made according the method described by Machinnonic, the animal were anesthetized using an abdominal injection of ketamine (0.3ml/100g). The procedures were conducted using a surgical microscope under 10 magnifications and by the same person. The right sciatic nerve was exposed and identified. Its average diameter is 1.25mm. A 5mm length of silastic tube with an internal diameter of 1.2mm was cut longitudinally and placed around the nerves. The cut of tub was sewn closed with 7-0 sutures and the tub was reconstituted to its original diameter. With the growth of the nerves , the sciatic nerves will gradually be compressed. After the model was done, 60 healthy female SD rats (150- 200g) were divided into two groups randomly.Every group have 30 rats. After 3 months, we loose the compressed nerves, observe the right sciatic nerves , measure its diameter of the proximate,the distal end and the compressed and check the electromyogram.Under identical experimental conditions, the experimental group use an abdominal injectionof ginesenoside (200mg.kg-1.d-1 ) for one month.Meanwhile,the control group use an abdominal injection of equal sodium chloride for onr month.We observe the nerves in 4 weeks,8 weeks and 12 weeks after loosing the compressed nerves.There are 10 rats randomly of every group in every operation. The electhophysiological and morphometric studies were carried out at the 4th, 8th and 12th weeks after the operation.Results: The silastic tube of the compressed nerve grafts was wraped up by the soft connective tissue at 90th day after the sciatic nerve was compressed. But there was no adherence between them. After the silastic tube was taken out , the compressed nerve grafts became thin , its texture became harden and the amphi-nerve became thick. The change to look like neuroma was observed and the color of its surface was white. At 4th, 8th, 12th week after the operation, the diameter of the compressed grafts became thick, the thicken grafts of the amphi-nerve became thin gradually. The compressed nerves grafts in the experimental group became thick transparently. At 4th , 8th, 12th week, there was marked difference between two groups. At 3rd month after the sciatic nerves were compressed, the nerves conduction velocity decreased from 51.94±1.03m/s to 14.10±0.82 m/s. The nerve conduction velocity of two groups were both improved in the differential degree after the operation. But there was marked difference between two groups. As for the nerve regeneration,great number of nerve fibers and Schwann's cells with regular arrangement and regenerated myelln sheath were observed 4 weeks after using an abdominal injection of ginesenoside. After 3 months, compressed nerves with abdominal injection of ginesenoside regenerated and restored to normal,while in the control group,only a small number of nerve fibers and Schwann's cells with irregular arrangement and rare regenerated myelln sheath were observed. The result showed that the electrophysiological indices, the diameter and the myelin sheath thickness of regenerating nerve fibers, the degree of the connective tissue decreasing were much better in experimental group than in control group.Conclusions: After peripheral nerve was compressed,it might cause serious partial pathological changes. These changes might be aggravated as the time of compression was prolonged and the compression force was increased. It was concluded that for the peripheral nerve compression disease,decompression should be done as early as possible in order to avoid or minimize the peripheral nerve injuries. Extensive epineurium solution and resection will have broken down the blood circulation and nourishment of the nerve and aggravated the partial scar formation and adhesion. It is disadvantage to the regeneration of the nerve. The relation of the nerve's regeneration and the blood circulation and nourishment of the nerve is closed. Therefore, it can accelerate the nerve's regeneration to relieve the compression and to improve the blood circulation and nourishment of the nerve.After total spaonins of panax notoginseng applies to the compressed nerves,it can accelerate vascular regeneration in the nerves and near the nerves.Accordingly, it can promote the transport of nourishment , reinforce the growth capability of Schwann's cells and nerves.Total spaonins of panax notoginseng lessens inflammatory reaction of the compressed nerves and expends the blood vessel which promote remarkable nerve's regeneration and functional recovery. The price of ginesenoside injection is cheap and its remarkable effect,so it is easy to clinical generalization .
