Objective: to get a systematic evaluation about therapeutic effect, influence onphysiological functions, incidence of ADRs of Caspofungin and to investigate theeffect on the blood level of Tacrolimus, a immunosuppressant. Method: Acase-control study enrolled 100 patients after transplantations with fungi infection,who were administered one of Caspofungin, Fluconazol or Amphotericin B. Thepatients who were administered Caspofungin were enrolled in test group. The patientswho were administered Fluconazol were enrolled in control group 1. The patients whowere administered Amphotericin B were enrolled in control group 2. The data ofphysiological index, ADRs complaint and symptom, blood level of Tacrolimus wererecorded on time. The date was handled statistically by SPSS 14.0. Result: Theaverage age of patients in 3 groups were 55.5±8.4, 53.6±11.3 and 45.3±18.9,respectively. The effective rate of 3 groups were 76.5%, 81.3%and 66.7%,respectively. The physiological indexes between 3 groups before administration havenot significant difference. ALT level of test group of before and after administrationwere 242.6±141.5 and 86.3±66.8 IU·L-1 respectively. ALT level of control group 2 ofbefore and after administration were 259±224.5 and 61.8±74.5 IU·L-1 respectively.ALT level of test group and control group 2 have significant difference between proand post- administration. BUN level of before and after administration of controlgroup 2 were 13.26±9.11 and 17.29±2.99 mmol·L-1. And the difference has statisticalsignificance. Blood level of K+ in test group increased significantly afteradministration. Blood level of Na+ in control group 2 decreased significantly afteradministration. Blood levels of Tacrolimus before administration of 3 drugs were5.92ng·mL-1, 3.79 ng·mL-1 and 2.91 ng·mL-1, respectively. Blood levels of Tacrolimusduring administration were 5.22 ng·mL-1, 5.53 ng·mL-1 and 3.23 ng·mL-1,respectively. Blood level of Tacrolimus in control group 1 increased afteradministration. The incidence of ADRs in three groups were 5.6%, 5.6%and 14.3%,respectively. Conclusion: Caspofungin has equivalent therapeutic effect comparedwith Fluconazol and Amphotericin B. And it does not induce the change of serum level of Cr, BUN, AST, ALT and TBIL. The incidence of ADRs was lower thancontrol group 2.
