Objective: To discuss on renal Hemodynamics in patients with chronic obstructive pulmonary disease and chronic Respiratory Failure and to assesses value of the blood flow spectrum of renal artery in reflecting early impairment of renal function in patients with chronic Respiratory Failure.Methods: the blood flow spectrum of main renal arteries segmental arteries interlober arteries were measured in 50 chronic obstructive pulmonary disease and 65 chronic respiratory failure and 44 healthy controls by color Doppler ultrasonography.Results :In chronic respiratory failure with chronico obstructive pulmonary disease renal hemodynamics of the blood flow spectrum was statistically different from that in chronic obstructvie pulmonary disease and the control group (P<0.05), The resistant indexes (RI) of all the renal arteries were increased significally acompared with those of the nomal group (P<0.01) and chronic obstructive pulmonary disease (P<0.01) in chronic respiratory failure. The Vd,Vs were lower that those of the normal group (p<0.01) (p<0.05) and chronic obstructive pulmonary disease (p<0.01) (p<0.05), in chronic respiratory failure, while no different could be found at the levels of serum creatinire and urea nitrogen between the three groups (P>0.05), in chronic obstructive pulmonary disease all renal hemodynamics of the blood flow spectrum was not statistically different from that in the control group (p>0.05).Conclusions: The impairment of renal hemodynamics in chronic respiratory failure patients can be determined quantitatirely by color Doppler ultrasonography before the elevation of serum creatinire and urea nitrogen The blood flow speetrum of renal arteries is a sensitive index which can reflect the early impairment of renal function in chronic respiratory failure patients.
