PurposeBy sepsis pathological processes, sepsis, severe sepsis and Septic shock, the time patients were pointing wei qi ying xue, Do the acute physiology and chronic health evaluation II and determination of the different time in patients with advanced inflammatory mediators high mobility group protein (HMGBI) content, then the related analysis aimed at understanding the qi fen, ying fen, xue fen sepsis patients uncontrolled inflammatory response characteristics of the law of the state, to further clarify the pathogenesis of sepsis. Discussion on combining traditional Chinese and western medicine treatment of inflammatory intervention laid the groundwork for a program to decrease the mortality of sepsis.MethodsWill be in April 2004-2006, in our hospital ICU and the fifth internal medicine collected with the diagnosis of sepsis 24 patients, into the branches within 24 hours by the weiqiyingxue differentiation, into a qifen, yingfen, xuefen group the eight cases, located eight cases as a normal control group, while 24 patients for the APACHEII score, A sample of patients and normal peripheral blood injected, centrifugal from serum, Application western blot assay Detecting all serum high mobility rate B1 protein (HMGB1) content, for the analysis.Result1. The sepsis patients with APACHEII scores: the xuefen grouping score higher than the yingfen, a higher qifen group (P<0.01). Application Spearman correlation analysis. The theory of weiqiyingxue and the APACHEII was positively correlated (P<0.01). 2. The control group and patents with sepsis HMGB1 content: qifen is higher than the control group(P<0.05); the yingfen group is higher than the control group(P<0.01);the xuefen group is the same (P<0.01); a higher yingfen group than qifen group (P<0.05); the xuefen group is higher than yingfen group (P<0.01); yingfen group and xuefen group was no significant difference (P>0.05). TCM show type, and from qifen→yingfen→xuefen, the HMGB1 levels increased gradually. Application Spearman correlation analysis: the differentiation period of weiqiyingxue and HMGB1 was positively correlated (P<0.01).3. APACHEII score and HMGB1 correlation analysis: Application pearson correlation analysis: HMGB1 and the APACHEII were positively correlated(P<0.01).4. 28-day mortality compared: the qifen group 0% (0/8) with the yingfen group 12.5% (one/eighth), no significant difference (P>0.05); with the xuefen group 50% (4/8), There was significant (P<0.05); the yingfen group with the xuefen group, no significant difference (P>0.05). the xuefen group showed a higher mortality rate than gas group.5. Death group and the survival group HMGB1 content and the APACHEII score comparison: died of HMGB1 content higher than the survivors (P<0.01). Died of APACHE II score higher than those who survived (P<0.05).Conclusions1. Sepsis is a Chinese "fever" areas. Sepsis pathological process of sepsis, severe sepsis, septic shock and the weiqiyingxue differentiation from qifen→yingfen→xuefen basically the same process. Note in clinical work, will be doing the theory of weiqiyingxue used Syndrome sepsis.2. HMGB1 as a late inflammatory mediator involved in the pathogenesis of sepsis. and sepsis in the pathological process in various stages of HMGB1, the more severe the disease, the higher the mortality rate is greater. Note HMGB1 very good response to sepsis Pathological evolution process, Sepsis can judge the condition and prognosis of clinical indicators.3. Correlation analysis showed that the theory of weiqiyingxue and the APACHEII score, HMGB1 there is a positive correlation between. The qifen septic patients, the APACHEII score low, HMGB1 levels are also low, the mortality rate is low, yingfen and xuefen patients, APACHEII score, HMGB1 high content and high mortality. Clinical will be a combination of all three, will contribute to a more accurate judgment of sepsis condition and prognosis.
