ObjectiveTo investigate the relationship between molecular markers : LUNX,CK19,CD326 and occult lymph node micrometastasis in NSCLC, and to reevaluate N-stage of NSCLC in molecular level that may help to predicting prognosis and decision-making for adjuvant therapy after radical operation in the future.MethodsThere were 23 NSCLC patients without adjuvant chemotherapy and 3 patients with benign diseases underwent surgical resection. 19 Lung cancer tissues, 116 groups lymph node from NSCLC patients and 11 groups lymph node from benign lesion patients were detected. Each sample was divided into two parts, one for conventional pathology examination, the other for detection LUNX,CK19,CD326mRNA to determine occult lymph node micrometastasis. All data were analyzed by chi-square test.ResultsLUNX mRNA was observed in all lung cancer tissues, and was observed in lymph nodes in 22 of 23 NSCLC patients by nested RT-PCR. LUNX mRNA was detected for in 74 groups in 22 patients including 14 groups in 5 patients with lymph node metastasis diagnosed by HE staining. There was no LUNX mRNA expression in lymph nodes of benign pulmonary lesion. Micrometastasis rate in groups by HE staining and RT-PCR for LUNX mRNA were 12.07%(14/116) and 63.79%(74/116), and in patients were 21.74%(5/23)and 95.65%(22/23). Interestingly, all samples including tissue of NSCLC patients,lymph nodes of NSCLC patients and lymph nodes of benign pulmonary lesion were detected by nested RT-PCR for CK19 and CD326 mRNA.ConclusionsIt is helpful to diagnosis occult lymph node micrometastasis by detecting the expression of LUNX in lymph node of NSCLC patients. It may be a promising way to reevaluate N-stage of NSCLC, help to predicting prognosis and decision-making for adjuvant therapy after radical operation. In this study, the evaluation of CK19 mRNAå’ŒCD326 mRNA expression fail to exactly predict occult lymph node micrometastasis for they were also observed in lymph node without micrometastasis.
