The Study Of Diagnosing Early-staged Lung Cancer By Dynamic Enhanced Computed Tomography Scanning

【Objective】To study the character of dynamic enhanced CT in early-staged lung cancers.And to find the relationship between the other SPNs(D≤2cm) whose feature are similar in CT.To study the value of diagnosis and limitation and differential diagnosis of the early-staged lung cancer and the similar SPNs.【Methods】The SPNs(1.0cm～2.0cm)which were found in pro-contrast CT and no transference were thin-scanned and analysed the CT values.And in a week these SPNs have dynamic enhanced computed tomography scanned and have pathological proved. All SPNs have dynamic enhanced on 20″,1′20″,2′20″,3′20″,4′20″after injection of 100ml contrast material(3ml/s).To see the SPNs′type of enhancement and analogy the limitation of enhancement.【Results】76cases in our study which have thin-scanned with dynamic contrast have 47 cases proved by surgery or percutaneous pulmonary puncture biopsy, the early-staged lung cancers(28cases), tuberculomas ( 8 cases ) , inflammatory nodules (10cases) and bronchocele (cases 1). The early-staged lung cancer had no significant difference from inflammatory nodules in the dynamic enhanced limitation (p＞0.05).And the enhanced type is similar.But the maximum limitation of enhancement and the time of reaching peak height have value.All early-staged lung cancers which are approved have been significantly enhanced besides one case which has no enhancement can be divided into two types:whole enhancement in 23 cases and centerd enhancement in 4 cases.【Conclusion】Dynamic CT enhanced scanning is very valuable to both diagnosis and distinguish diagnosis of the SPNs.As postcontrast- enhanced CT number increase＜25Hu,it is the threshold of the benign nodules.The enhancement limitation early-staged of lung cancer is not very different of inflammatory nodules and it is not easy to distinguish them.But if the enhancement limitation of SPNs≥60HU ,it is very possible of inflammatory nodules.
