Objective: To determine if different methods of operation and preoperative mental state will influence the incidence of early postoperative cognitive dysfunction(POCD) after non-cardiac surgery in the elderly.Methods: One hundred and twenty elderly patients were randomly allocated to undergo elective abdominal surgery either laparotomy or laproscopic operation under general anesthesia .Cognitive function was assessed preoperatively and 4 days postoperatively using MMSE, preoperative depression measured by SDS and preoperative anxiety by SAS as well. A postoperative dysfunction was defined when decline by more than or equal to 2 scores below the preoperative value of that test.Results: A hundred and fourteen patients completed both preoperative and postoperative neuropsychological tests. On day 4,POCD occurred in 12/57 patients(21.05%)in the laparotomy group and in 10/57 patients(17.54 % ) in the laproscope group. No significant difference was found between the two groups. Logistic regression analysis showed that short duration of education and age were independent risk factors of the occurrence of early POCD.Conclusions: Early POCD occurred in 19.298% of the gerontal patients undergoing abdominal surgery. However. there was nosignificant difference between the effects of two different methods ofoperation on the incidence of POCD. Short duration of education and agewere risk factors of early POCD.
