Objective: To study on the correlation between MSCT features and expression of COX-2 and VEGF, MVD in clear renal cell carcinoma.Methods:â…§- related antigen, expressions of COX-2 and VEGF in 32 patients with CRCC diagnosed by pathology and 10 normal renal tissues were examined immunohistochemically using S-P techniques. Multi-slice CT examinations were performed with PHILIPS BRILLIANCE 16 CT scanner in 32 patients. After unenhanced CT scans were performed, contrast medium(Iohexol Injection 300mg/ml) (1.5ml/kg. weight) was injected intravenously with a power injector at a rate of 3ml/s. Three-phase enhanced MSCT scans were obtained at 25s(corticomedullary phase, CMP), 90s(nephpographic phase, NP) and 5min (excretory phase, EP) respectively after the beginning of injection. The scan parameters were 120KV, 250mAs, collimation 5mm , pitch 1.5. CT images were observed for tumor size,intratumor necrosis, liquefaction, cystic degeneration, tumor margin, the density on unenhanced scan, CMP, NP and EP, the maximum enhancement extent, invasion of renal vein or/and inferior vena cava, invasion of adjacent organs or/and distant metastases and compared with immunohistochemical result.Results: 1 The positive expression rate of COX-2 and VEGF, MVD in CRCC was significantly higher than that in normal renal tissues(p< 0.01); 2 As nuclear grade increased, CRCCs were more likely to be positive COX-2 and VEGF expression , MVD(p<0.05); 3 the density on CMP, NP, EP and maximum enhancement extent in CRCC showed a positive correlation with the expression of COX-2 and VEGF, MVD; 4 The positive expression of COX-2 and VEGF,MVD were significantly correlated with tumor size, intratumor necrosis, liquefaction, cystic degeneration, tumor margin, invasion of renal vein or/and inferior vena cava, invasion of adjacent organs or/and distant metastases(p<0.05), but not related to the density on unenhanced scan(p>0.05); 5 The expression of COX-2 and VEGF,MVD showed a positive correlation in CRCC(related parameter is 0.609, 0.739, 0.826, respectively, P <0.01).Conclusions: 1 the density on CMP, NP, EP and maximum enhancement extent in CRCC showed a positive correlation with the expression of COX-2 and VEGF,MVD,which indicated the enhancement features by MSCT could reflect the condition of angiogenesis in tumor. 2 The positive expression of COX-2 and VEGF, MVD were significantly correlated with partial CT features in CRCC, so that MSCT may be a noninvasive method to predict the biological behabior and malignance of CRCC at cell biological level to some extent. 3 The positive expression rate of COX-2 and VEGF, MVD in CRCC was significantly higher than that in normal renal tissues; moreover, CRCCs were more likely to be positive COX-2 and VEGF expression , MVD with nuclear grade increasing.
