Clinic Study Of ～(32)P-glass Microballoons Interventional Treating Primary Hepatic Carcinoma And Meaning Of Detecting Telomerase

Objective:To investigate ³²P-glass microballoons interventional embolotherapy combining with hepatic artery perfusion of primary hepatic carcinoma through hepatic arterial infusion and to observe the change of hTRT(human telomerase reverse transcriptase)activation of peripheral blood after therapy.Methods:60 cases who were diagnosed as primary hepatic carcinoma in intermediate stage or advanced stage from October 2005 to April 2007.Were divided into two groups.30 cases as treatment group.Among these cases,25 cases were men and 5 cases were women and the age fluctuated between 41 and 74 years old.The average of age is 54.5±13.5 years old.20 cases were single tumor and 10 cases were multiple tumor.On the base of type,the number was 13,11and 6 respectively in massive,nodular and suffusion type.20 cases were concurred secondary tumor and metastasizing of inside liver and 4 cases were complicated tumor embolus in vein.18 cases were suffered from type B hepatitis combined with hepatic cirrhosis in different extent.All the 30 cases showed clinic symptom in different degree and 21 cases combined with AFP increasing. All the cases have indication and contraindication.All the cases were treated through arteria hepatica infusing embolotherapy and chemical therapy by ³²P-glass microballoons combining with embolic agent combining with hepatic artery perfusion.The other 30 cases(group B)by TACE simultaneously as controls.The average age,diameter of tumor and rate of metastasis were similar with two groups(P＞0.05).Two groups correlative image tests,blood routine tests, liver function tests,a -fetoprotein and peripheral blood hTRT activation was detected 1 month before and after operation and positive rates was obtained by comparing with normal values.The results of before and after operation was compared respectively.Investigate the clinic effective.Results:A groups clinical symptomatic relief rate was 50%after ³²P-glass microballoons interventional embolotherapy combining with hepatic artery perfusion.The B group was 30%.In the A group 15 cases' tumor(50%)deflated in different extent:and 7cases(23.33%)deflated beyond 50%,8 cases(26.67%)deflated 25%- 50%and 9 cases(30%)did not change.6 cases (20%)enlarged.In the B groups 9 cases' tumor(30%)deflated in different extent:and 4 cases(13.33%)deflated beyond 50%,5 cases(16.67%)deflated 25%- 50%and 12 cases(40%) did not change.9 cases(30%)enlarged.The two groups change of blood routine and liver function was not significant after treatment.The positive rate and density of AFP was decreased significantly.The treatment group were much less those in control group after treatment.The positive rate of hTRT activation of hepatoma cases which was remarkable higher than the normal cases,and it was decreased remarkably after treatment,The treatment group were much less those in control group after treatment.Conclusion:³²P-glass microballoons interventional embolotherapy through hepatic arterial infusion is a kind of safety,effective and feasible method,especially in treating primary hepatic carcinoma cases with rich blood supply.The clinic effect is significant when it combines with chemical therapy.Peripheral blood hTRT activation of primary hepatic carcinoma cases was decreased remarkably after ³²P-glass microballoons interventional embolotherapy through hepatic arterial infusion.It can be judged that tumor occurring,developing,local recidivating and advanced stage of hepatic carcinoma cases by periodical testing hTRT activation after interventional treating.
