Objective:Febrile seizures(FS)refers to the seizure happens to the children with rectum temperature above 38℃,from 6 months to 5 years old,had whose nervous system functions normally,without definite intracranial infection or case history.FS is a kind of disease frequently happened to children,doing great harm to the brain.Children under 5 years old take up a percentage of 2%-5%,in which one out of three have a relapse,one out of nine suffer from the disease three times or even more.For the time being,the mechanism of FS is still unknown.However,during the recent years,research on animals shows that the HO/CO system could change a little bit when FS happens.CO,recently discovered as a new type of molecule transmitting gaseous message,could lessen the injury to the cells,dilate blood capillary and improve blood circulation.Original CO,as a new type of nervous transmitter,involves in the adjustment of the damage to the brain.Research on the mechanism that FS dose harm to the brain shows that HO/CO system which plays a great role in central nervous system through the animal test.But dose the result based on animals apply to children? So far there is no report of this kind.This study is to observe the change of heme oxygenase-1(HO-1)activity and endogenous carbon monoxide in children with Febrile Seizure.Methods:The thirty cases were provided by department of pediatrics attached to Shanxi People's Hospital,20 cases for male,and 10 cases for female,aging from 6months to 5 years,with an average of 2.5±0.6 years old.All of them had no nervous disease,and 25 cases had family history.After seizures 2 hours and 48 hours Venous blood was collected from children with FS,including 30 children with FS and 15 healthy children who were enrolled to serve as normal controls.The levels of HO-1 activity;COHb were measured with the double wavelength scanning by a spectrophotometer.Results:The HO-1 activities 83.07±7.03 nmol/hL and COHb 2.73±0.39%in children after 2 hours with Febrile Seizures;the HO-1 activities 34.20±4.68 nmol/hL and COHb 1.18±0.41%in children after 48hours;the HO-1 activities 30.20±5.91 nmol/hL and COHb 1.13±0.33%in normal children.There were distinct difference between subjects after seizures 2 hours and normal children; also distinct difference between subjects after seizures 2 hours and 48 hours (P<0.01);no difference between subjects after 48 hours and normals(P>0.05); The levels of COHb in FS children also elevated as compared with normal control subjects(P<0.01),and correlated to the level of HO-1 activity respective ly(r=0.804).The above research suggests the followings:1.Two hours after seizures,HO-1 activates positively,showing that HO-1 takes part in the mechanism of FS.HO-1 was the induced form of HO when FS occurs,there was a lack of oxygen.Under various kinds of influence,oxidation and inflaming cells for instance,HO-1 Mrna is enabled.Thus,HO-1 protein synthesizes and activates.2.The average level of CO changed according to HO-1 activity,which telld that there were positive relationships in between.CO produced by the decomposition of heme in HO system was the only source of original CO. Meanwhile,HO-1 was the induced form of HO.The increase of original CO was probably caused by the increase of HO-1 creativity.Conclusions:This study illustrated the change of endogenous HO-CO system in children with FS,then to calculate that endogenous HO-CO might take part in FS,deepen the knowledge of the harm that FS do for the brain,and to provide clinical bases for exploring the measures dealing with the harm.
