Objective: The determination of Platelet membrane GPⅡb/Ⅲa and platelet agglutination test (PAgT) is very important in the diagnosis of cardiovascular disease. The activation of GPⅡb/Ⅲa acceptor is the final and common pathway in platelet aggregation. The platelet aggregation plays a major role in thrombogenesis. Platelet activation is one of the etiopathogenesis on many diseases; it plays a major role in acute coronary syndromes (ACS) and in thrombotic complications during and following percutaneous coronary interventions (PCI). Prior studies have shown that GPⅡb/Ⅲa plays a major role in thrombocytic agglomerate and release reaction. The advance of GPⅡb/Ⅲa is the important sign in thrombocytic adherence and aggregation, and is one of the most important factor in thrombogenesis. GPⅡb/Ⅲa can reflect the activated state of platelet directly. The advance of quantity in GPⅡb/Ⅲa is the important reason of the advance of platelet adhesion. We can comprehend the extent of platelet activation through the detection of GPⅡb/Ⅲa. Detection of platelet aggregation not only reflected the state of blood coagulation in disease, but also provided valid information in CHD invasion and mechanism. So the detection PAgT could be used in many diseases besides assisting diagnosis in acute myocardial infarction and cerbral infarction. PAgT was not the specific diagnose criteria but it usually had close relation to severity level of disease and using indic of anticoagulant drug and prognosis decision. It had some assistance to formulating therapeutic regimen also. The purpose of the present study was to investigate the influence of random lunch on platelet activation and compare the difference influence on platelet activation between ASA + LMWH and ASA + clopidogrel + low molecular weight heparin (LMWH) by measuring platelet membrane GPⅡb/Ⅲa and PAgT .Methods: 90 patients were enrolled between February 2006 and December 2006, including 69 patients with coronary heart disease 17 patients with arrhythmia 2 patients with inborn heart disease and 2 patients with valvular heart disease. There were 54 males and 36 females, with a mean age of 58.01±14.66years(21-82). Patients were divided into compare group, ASA + LMWH group and ASA + clopidogrel + LMWH group according to whether or not using entric coated aspirin, clopidogrel and LMWH. All of the patients in three groups were never received any anticoagulation treatment one month before hospitalization. All of the patients in compare group were new patients, blood samples were obtained to detect GPⅡb/Ⅲa and PAgT before and 1hour, 2hour after random lunch on the day admission. In the other two groups of patients, blood samples were obtained to detect GPⅡb/Ⅲa and PAgT before and 1hour, 2hour after random lunch on the medication seventh day. Measurement datas were expressed as the mean value±standard deviation (SD). The comparison among the three groups and the comparison in each group were evaluated by repeated measures engineered variance analysis. A p value of <0.05 was considered statistically significant.Results: The baseline characteristics such as age, body weight, gender, smoking, history, hypertension, hypercholesterolemia, diabetes mellitus were not significantly different among the three groups (p>0.05). GPⅡb/Ⅲa and PAgT before and 1 hour, 2hour after lunch among compare group, ASA + LMWH group and ASA + clopidogrel + LMWH group were not significantly different (p>0.05). It indicated that random lunch had no manifest influence on GPⅡb/Ⅲa and PAgT. The results of PAgT: there was significantly difference before and 1hour, 2hour after lunch between compare group and ASA + LMWH group; compare group and ASA + clopidogrel + LMWH group ( P<0.05 ) . There was no significantly difference between ASA + LMWH group and ASA + clopidogrel + LMWH group(P>0.05), ASA + LMWH group was higher. The results of the expression of platelet membrane GPⅡb/Ⅲa: there was significantly difference before and 1hour, 2hour after lunch between compare group and ASA + LMWH group, compare group and ASA + clopidogrel + LMWH group ( P<0.05 ) .There was no significantly difference between ASA + LMWH group and ASA + clopidogrel + LMWH group(P>0.05),ASA + LMWH group was higher.Conclusions: The present study demonstrated that random lunch had no significant influence on GPⅡb/Ⅲa and PAgT. There was no significantly difference to PAgT between ASA + LMWH group and ASA + clopidogrel + LMWH group.
