Objective:through extrusion thrombus and Fogarty balloon catheter embolectomy two difficult methods to heal rabbit acute deep vein thrombosis of the lower extremity,detect the patency rate of the vein,the morphologic change of the wall of vein and the change of TF,TXA2/6-keto-PGI2 whiich can reflect the activity of endothelial cell,to choice suitable surgical thrapy.Methods:There are 80 experimental rabbits,which were 2.5~3.0kg weight.Choice 50 rabbits from the total randomly after setting up the thrombotic model in the femoral vein and divied into A,B groups(Group A and B).The other 25 were only cut opened the skin and not to set up the model(Group C).And another 5 were normal group(Group D).Group A were treated by the extrusive method and group B were treated by the Fogarty balloon catheter embolectomy. Make angiography at specific points,which are 7 days,14 daysand 28 days after operation,to compare the patency rate of the vein.Collected the sample of the thrombotic section and the same section in Group C on 1 day,4 days,7days,14 days and 28 days.The morphological change of venous wall were assessed by morphological scanning electron microscopy and transmission electron microscopy.The mRNA levels of TF were analyzed by RT-PCR.The contents of the TXA2 and 6-keto-PGI2 were detected with ELISA.Results:The angiography found the femoral vein were fluency in Group C and D in 7 days ,14 days and 28 days after operation..In Group A,one sample was found blood current tenuity at 14 days,one was occlusive in the femoral vein at 28 days ,the blood current ran into the vena cava posterior through the branch stem.The others were normal .And in Group B,there were one sample in each phase obliterative in the femoral vein because of relapse .For scanning electron microscopy result,Group A had a process which the injury of endothelial cells were from lower-grade to repair. In Group B,the injury of endothelial cell reached a peak from 4 to 7 days ,gradedⅣ.And Group C,the injury was silm,,and restored to normal at 28th days.;For transmission electron microscopy,Group A was found vacuolar degeneration in the endochylema at the first day,the vacuolus was more apparently on days 14.The partial endothelial cells colobomaed, ground substance under the endodermis exposed.The vacuolar degeneration was the main change in Group C ,and then repaired gradually.TF- mRNA were not expressed in Group D.There were a process of ascensus and degression,reach the peak between 7 and 14 days.Compared with Group D respectively,there were significant difference(P<0.05).Group A compared with Group B, there were significant difference(P<0.05) too.TXB2 expressed in Group A,B and C hightened apparently compared with Group D,Group B was most highest and Group A was at the second.In each group , TXB2 first stepped up and then degraded.Contrary to TXB2, 6-keto-PGF1αin Group D was most highest.There were significant difference (P<0.05) at the 4th day and 7th day among groups.Conclusion:1) Compared with Fogarty balloon catheter embolectomy, extrusion embolectomy can discard the thrombus more thoroughly and guarantee the patency rate of bloodstream in animal experiment.But weather it is the same in the clinical,we will rearch and confirm in the clinical part of the topic.2) Both extrusion embolectomy and Fogarty balloon catheter embolectomy had damage to the blood vessel endothelium to some extent.The injur was serious between 7 and 14 days,and then began to repair from 14 days.Compared with extrusion embolectomy,the later made more damage to the blood vessel endothelium.and repaired less completely.3) On biologic activity of the vascular endothelial cell,Group B embodied more activity,and realeased more tissue factor and TXA 2,thus add the rate of the relapse to some extent.
