[Objective:] [1]To develop spine external transpedicular fixation technique, A new type of percutanous external transpedicular fixation(PETF) used to therapy spine fracture has been designed.[2]to analyze its clinical feasibility and significance associated with PVP,The biomechnical test has been finished.[3]To introduce a minimally-invasive technique of a new percutanous external spinal fixation assistanted minimally-invasive surgical procedure treating thoralumbar fracture and analyse its clinic applicable area and value.[Methods][1]According to advantage of spine external transpedicular fixation technique and limitation of traditional percutanous external transpedicular fixation,design a new percutanous external transpedicular fixation.[2]A spine fracture model was made,the experimentation was to test the three-dimensional stabilization according to the order of normal group(A), spine fracture model group(B), fixed by PETF group (C), fixed by PETF associated with PVP group (D). And analysed according to statistics way.[3]there were sixty-nine cases of thoralumbar fracture treated by PETF assistanted mini- trauma technique, from 10 to 70 age.thereinto,.26 caces were compression fracture because of osteoporosis,43 caces were compression or crack fracture because of trauma.all cases presented stenosis, neurological function are better than B class according to Frankel.[Result][ 1]the three-dimensional stabilization of group A is better than group B, P<0.001, group C is better than group A,P<0.001, there is no markble difference between group C and group D. P>0.5, But group D is better than group C in axes compressing, P<0.001.[2]all cases had no post-operation syndrome , the pain syndrome was distinctive lightened, anterion height of centrums came back and spine stenosis was reliveved. neurological deficit was definitely improved according to Frankel. the PETF were installed for 10 days to 3 months after operation.the clinic effect were satisfaction except for 9 patients suffered bolt- hole infection and 3 centrum height losed ,1 bolts loose.[Conclusion ] [1]The spine fracture model is instability model,it is true of this biomechnical test.[2]The spine fracture model is a type of instability spine model, it is satisfacted for the test. The new PETF can offer good three-dimensional stabilization to instability fracture spine.it has clinical feasibility .[3]To be a minimally-invasive surgical procedure ,the PETF assistanted microendoscopy can combine decompression and reduction with external fixation for the injuried vertebral colunm without segments fusion.
