| ObjectiveCervical intraepithelial neoplasis (CIN) inclues cervical dysplasia and arci-noma in situ. The infection of human papilloma viruses (HPV) has great rela-tionship with the occurrence of CIN and cervical cancer. HPV16,18 makes greatefforce in the high-risk HPV genetics. It makes the development of CINâ…¡,â…¢and cervical cancer. HPV16,18E6 protains is oncogene which makes cell trans-forms as data says. We investigate 6251 patients who are from department of gy-necology in China Medical Univercity associated hospital from November, 2004to August,2005 with infection of cervix using liquid-based cyology test(LCT),while we also detect 86 CIN and 14 chronic cervicitis with the HPV16,18E6proteins, using Western blot which is the first in our country using cercix liquid-based cyology. The objective is to detect the affection of HPV, the correlationof CIN and the affction of HPV16,18E6,and the clinical siganificane of screen-ing of CIN using LCT and detecting of HPV16,18E6 proteins.Materials and Methods1. MaterialsWe investigate 6251 patients who are from department of gynecology in Chi-na Medical Univercity associated hospital from November,2004 to August 2005with infection of hpv and aff cavitas pelvisection of cervix using liquid-basedcyology test, LCT, age from 21-76 years, average 45.53 years. All patients arewithout operation on cervix, without radiotherapeutics on cavitas pelvis. The cases used to analysis of western blot are screened from LCT, and theyare grouped by the effort of histopathology , including 14 chronic cervicitis(con-trol group),24 CINâ… (CINgroup),30CINâ…¡(CINâ…¡group),32CINâ…¢(CINâ…¢group),The 100 case above, age from 23-57 ages, average 41.23 ages, include34 patients who are LCT negative.Antibodies are HPV16,18E6 and IgG-HRP, from Santa Company in USA.2. MethodsLiquid-based cytology test was used to investigate infectious frequency ofHPV and state of cervical lesions in 6251 patients from gynecological clinic serv-ice of the first affiliated hospital of China Medical Uiversity during Nov 2004 toAug 2005. Among them 100wemen(86 cases of CIN and 14 cases of chronic cer-vicitis) were test with western blot for the expression of HPV16,18E6 in the res-idue of cervical liquid-based cytology.3. Statistical analysisChi-square test was used in the analysis. P<0.05 has statistical signifi-cance.ResultsDetection rate of LSIL and above lesions was 15.3% (954/6251) in 6251cases detected by LCT. Infectious frequency of HPV was 4.8% (301/6251),which was distributed in LSIL lesions and koilocystosis positive rates decreasedsignificantly with the seriousness of cervical lesions. Using histopathology to test403 cases screened by LCT indicated that coincidence of LCT and pathologicalexamination increased significantly with the seriousness of cervical lesions. Co-incidence in LSIL,HSIL and CC were 52.8%,88.9% and 100%,respectively.Western blot was used to detect the expression of HPV16,18E6 in the residue ofcervical liquid-based cytology. The results suggested that infectious frequencyof HPV16,18E6 increased with the seriousness of CIN. The positive rate ofHPV1618E6 inCINâ… ,CINâ…¡,CINâ…¢and control group was 33.3% (8/24),46.7% (14/30),59.3% (19/32) and 7.1% (1/14),respectively. The positiverates of HPV16,18E6 in CINâ…¡and CINâ…¢were significandy different from con- trol group(P<0.05,P<0.01). CINâ… and CINâ…¡had no significant difference(P>0.05). The expression of HPV16,18E6 in 2 cases of CINâ…¡and 8 cases ofCINâ…¢missed diagnosis by LCT was tested by western blot and the results werepositive. However, we did not find parakeratotic cells in the cases tested byLCT.ConclusionHPV16,18E6 infection was closely associated with CIN. Cervical liquid-based cytology were tested using LCT and western blot, which was convenientand sensitive. It can increase detection rate of patients with CIN and it was ofsignificance to find out, monitor, interfere and treat those patients with CIN whowere infected by high risk HPV and defend CIN from malignant transformation. |
