Part One: Research on the Aesthetic Three-dimensional Anthropometry of Zygoma in OrientalsObjective:The purpose of this study was to analyze the use of references andlandmarks to assess the aesthetic three-dimensional position of zygoma and provide aesthetic basis for reduction malarplasty and reflect the malar eminence and its significance in the clinical application through the three-dimensional quantitative anthropometry of zygoma.Methods:12 patients who have protruding cheekbones with wide and short face in a front view and 14 native adults were examined by spiral volumetric CT(GE lightspeed).3D craniofacial bone images were reformatted and 3D measurements of zygoma were performed in SUN image workstation (Version AW 4.0).The data were explored and stratified into two group, patient versus native adult, and analyzed with the Independent-Samples t test and the Paired-Samples t test using SPSS version 13.0 software. A p value of 0.05 or less was considered statistically significant. Results:Measurements for the aesthetic three-dimensional position of the zygoma were accurate or reliable. The width of zygomatic arch and body ,the projection of zygomatic body and the thickness of zygomatic arch showed a marked change from patient to native adult. These were significant with the Independent-Samples t test (p<0.05).Analysis of symmetry did not achieve statistical significance with the Paired-Samples t test(p>0.05).Conclusions:These findings suggest that the three-dimensional quantitative measurements for zygoma describing the aesthetic three-dimensional position of the zygoma and reflecting the malar eminence are clinically useful. Part Two: Three-dimensional Midface Rejuvenation Combined with Zygoma Reduction in OrientalsObjective: To present a new effective method for correction of the changes of the midface with aging by combining zygoma reduction with three-dimensional midface rejuvenation. Methods: We retrospectively reviewed the treatment process of 11 patients with the changes of the midface with aging, evaluated the surgical designs and results of this method. Results: All patients were satisfied with the postoperative results. The well-known complications of asymmetry of the zygoma, facial nerve injury and paresthesia of the cheek were absent. In particular, because of the relatively reduced swelling, most of the patients could resume activities of daily life within 3-5 day after the operation.conclusions: Combining zygoma reduction with three-dimensional midface rejuvenation is a relatively ideal method for correction of the changes of the midface with aging.
