Objective:By Observing the testicle's form and structural change in children with bilateral high cryptorchidism under the naked eye and optical microscopy, explore the relationship between age and pathological changes of the Cryptorchidism testis.By measuring the levels of serum sex hormones in children with bilateral high cryptorchidism, which were compared with control group, discuss endocrine factors and hypothalamic-pituitary-gonadal axis roles in the pathogenesis of cryptorchidism.By comparing before operation group's levels of serum sex hormones with after operation groups, evaluate the curative effect of surgical treatment and provide the basis for clinical treatment.Method:24 boys with bilateral high cryptorchidism were divided into 1~2 year-old group and the 2~10 year-old group, and testes were first observed by the naked eye,and then 10 sides testicular of poorly developed or significantly atrophy were punctured, which was observed under optical microscope through conventional biopsy staining,Comparing the structural differences between the two groups.Serum testosterone(T),estradiol2(E2),follicle stimulating hormone (FSH)levels of 24 boys with bilateral high cryptorchidism were measured by chemiluminescence a week before operation and 3-6 months after operation. The results were as cryptorchidism before operation group and after operation group.Levels of serum sex hormones of 24 boys with indirect hernia were measured by the same method as a control group. Both compared among the three groups, the results were analyzed using statistical methodsResult:Both under Optical microscopy and under the naked eye are found: 2~10 year-old group undescended testicles were worse than the 1~2 year-old groups, and organizational structure changed significantly.Compared cryptorchidism preoperative group to control group with serum T,E2,FSH level, the difference is statistically significant.Compared cryptorchidism postoperative group to control group with serum T,E2,FSH level, the difference is statistically significant. The level of serum T was significantly lower than the control group, while the level of serum E2 and FSH were significantly higher than the control group.Compared cryptorchidism preoperative group to cryptorchidism postoperative group with serum T,E2 level, the difference is statistically significant. The level of serum T of postoperative group was significantly higher than preoperative group, the level of serum E2 of cryptorchidism postoperative group was significantly lower than preoperative group, while compared with serum FSH level, the difference is not statistically significant..Conclusion:1.After two years old testicle of boys with bilateral high cryptorchidism changed in the organizational structure, with the age, pathological changes gradually clear.2.The levels of serum sex hormones in children with bilateral high cryptorchidism were obviously abnormal, these hormones were synthesized and secreted by hypothalamic-pituitary-gonadal axis synthesis and secretion,or were subjected to regulation, which show endocrine factors and hypothalamic-pituitary-gonadal axis have played an important role in the incidence of cryptorchidism.3.After surgical treatment, the levels of serum testosterone and estradiol in children with cryptorchidism improved obviously;closed to the normal level.Illustrating curative recently effect of surgical treatment was reliable and was conducive to the recovery of testicular function.
