Objective:To investigate the expression of tumor suppressor gene maspin and tumor metastasis-associating gene CD44v6 in breast cancer and their correlation and their association with the expression of ER,PR,C-erBb-2 in breast cancer and clinopathological parameters,then their clinical significance were analyzed.Methods:The expression of Maspin and CD44v6 were determined by immunohistochemistry technique(S-P method)in 65 cases of breast cancer. Simultaneously,12 cases of breast benign lesion(including 6 cases breast fibroadenoma and 6 cases breast proliferation)as validative controls. Combined the expression of ER,PR,c-erbB-2 in breast cancer with the clinopathological parameters including age,tumor size,tumor histological types and grades,clinical stages,axillary lymph nodes and distant metastasis,the clinical significance of CD44v6 and Maspin were comprehensively analyzed.Results:(1)The expression of CD44v6 was all negative in 12 cases of breast benign lesion.Of the 65 breast cancer patients,64.6%(42/65)were CD44v6 positive,and CD44v6 expression in axillary lymph nodes metastasis was obviously higher than that in non-lymph nodes metastasis(100%vs 25.8%, p<0.05).The positive rate of CD44v6 was 9.1%(1/11),61.8%(21/34),100% (20/20)in histological gradeⅠ,ⅡandⅢrespectively(p<0.001).(2) The expression of Maspin was all positive in 12 cases of breast benign lesion. Of the 65 breast cancer patients,30.8%(20/65)were Maspin positive.Maspin positive expression in axillary lymph nodes metastasis was obviously lower than that in non-lymph nodes metastasis(30.8%vs 55.9%,p<0.05)The positive rate of Maspin was 42.2%(19/45),5.0%(1/20)in ER positive and negative cases respectively(p<0.05);The positive rate of Maspin in invasive duct cancer was lower than that in intraductal cancer(13.5%vs 100%,p<0.05). The positive rate of Maspin was 72.7%(8/11),26.5%(9/34),15.0%(3/20)in histological gradeⅠ,ⅡandⅢrespectively(p<0.05).(3)Spearman correlation analysis showed that the expression of CD44v6 was obviously correlated to axillary lymph nodes metastasis(r=0.195,p<0.05)and histological grade(r=0.593,p<0.001).The expression of Maspin was obviously correlated to clinical stages(r=-0.247,p<0.05),axillary lymph nodes metastasis(r=-0.194, p<0.05)and histological grades(r=-0.326,p<0.05).Meanwhile Spearman correlation analysis discovered that the expression of CD44v6 was obviously correlated to the expression of Maspin(r=-0.469,p<0.001).(4)Logistic multivariate regression analysis showed that the factors affected the expression of CD44v6 were axillary lymph nodes metastasis(P<0.05,B=38.200),tumor size(p=0.050,B=-1.545)and histological grades(P<0.001,B=3.146). Simultaneously regression analysis discovered the factors affected the expression of Maspin were axillary lymph nodes metastasis(P<0.05,B=-0.422),ER(P<0.05,B=-1.380)and histological grades(P<0.05,B=-1.244).Conclusions:(1)The expression of CD44v6 was obviously positively correlated to axillary lymph nodes metastasis and histological grades.The expression of Maspin was closely related to the status of ER in breast cancer. The expression of Maspin was obviously negatively correlated to clinical stages,axillary lymph nodes metastasis and histological grades.CD44v6 and Maspin were closely related to the prognosis of breast cancer.(2)The expression of CD44v6 was obviously negatively correlated to the expression of Maspin; increased expression of CD44v6 and decreased expression of Maspin possibly enhanced axillary lymph nodes metastasis in breast cancer.(3)Axillary lymph nodes metastasis,histological grades and tumor size were factors affected the expression of CD44v6 in breast cancer,then,axillary lymph nodes metastasis and histological grades both were risk factors.Simultaneously,axillary lymph nodes metastasis,histological grades and ER were factors affected the expression of Maspin in breast cancer,then,axillary lymph nodes metastasis and histological grades both were the factors down regulated the expression of Maspin.
