| Objectives:To evaluate the effect of early(24 hours after the model made)enteral nutrition(EEN) on the intestinal mucosa barrier in dogs with severe acute pancreatitis.Methods:SAP model was induced by injecting 1ml/kg of 5% sodium taurocholate into the pancreas via pancreatic duct and then practiced jejunostomy.15 dogs were divided into EEN group(n=8),PN group (n=7)randomly.The EEN group were practiced jejunum nutrition 24h after operation ,the PN group used amino acid ,fat milk, 50% glucose as main energy sources, combined with liposolubility and water-soluble vitamin.Each group kept for 3 days.The supply of heat,nitrogen and fluid were equal.The content of peripheral blood amylase and endotoxin were obtained before and 30 minutes after SAP ,1 and 2 days after enteral nutrion; The dogs were put to death after the experiment then we took liver ,spleen, pancreatic and lymphonodi mesenterici to identify the grow of the bacteria. We also observed the pathology of the pancreatic issue and give a mark to both group.We also take the ileum tissues to observe the mucosal morphologically through the light microscope and electron microscope.Result: the leve of plasma amylase and the plasma endotoxin elevated obviously after the model succeed , There were no significant difference betwen two groups in the data of plasma amylase. EEN significantly reduced the level of systemic plasma endotoxin 3 days after the model succeed compared with PN group ,but in the first two days there were no significant difference between the two groups.The positive rate of bacterial translocation to the pancreas and distant organ were obviously lower than the PN group.(P<0.05). There were no significant difference of the pathology score of the pancreatic tissue in the two groups.There were no significant difference of the mucosal morphous of the two groups.Conclusion:PN and EEN can not improve the pathology of the pancreatic tissue ,while EEN can significantly reduce the level of the plasma endotoxin to improve the systemic inflammation.enteral nutrition can improve the barrier function of the gut ,reduce the bacteria translocation. |
