Objective:To discuss the reasonable methods of surgical operation for differentiated thyroid carcinoma and the post-operational recurrence-related factors.Methods:To analyze retrospectively 172 cases of differentiated thyroid carcinoma after surgical operation, research the methods of surgical operation and carry through the follow-up visit, then observe the prognosis-related factors through the recrudescent cases.Results:The 4-year survival rates is 100%, and 15 cases of 172 following-up cases recurred. For the patients in III-â…£stages, the number of recrudescent cases with radical neck dissection is obviously lower than the number without radical neck dissection. Moreover, the recurrence rate of differentiated thyroid carcinoma in 4 years is closely related with age and clinical stages, while not related with sex. When the size of carcinoma localized in unilateral lobe is less than 1cm the recurrence rate is not related with the methods of surgical operation for the primary lesion. When the size is more than 1cm the recurrence rate of resection of pathological lobe and isthmus of thyroid gland is obviously higher than resection of hibateral lobes.Conclusion:The operational methods of differentiated thyroid carcinoma should been chosen according to the clinical stage,risk factor and lymph node metastasis. The operation of total thyroidectomy and prophylactic radical neck dissection for all of the patients is not right. The prognosis of differentiated thyroid carcinoma is closely related with age,clinical stage and so on.
