Arteriosclerosis obliterans (ASO) is a systemic disease. In limb arteries, the lower limbs arterial are easily to be put in to trouble, especially, the femoral artery shares the highest incidence rates. Some data indicate that the plasma ET of patients with ASO is significantly increased, and NO is significantly decreased, and has relationship with the ASO clinical stages, with the ASO condition increased, ET gradually increases and the level of NO decreases. At present, the color Doppler ultrasound has become the first choice of the clinical examination of arteriosclerosis obliterans because of its simple, real-time, accurate and other advantages. However, color Doppler ultrasound tests on the extent of lesions ASO grade, is not yet available relevant reports. In order to further study endothelin (ET), nitric oxide (NO) in atherosclerotic occlusive disease in the formation and development of the role and with the ASO color Doppler ultrasound in patients with lesions judge the relevance, we have adopted the ASO patients with high-frequency color Doppler ultrasound examination of lower limbs of patients with arteriosclerosis obliterans, analyse the two-dimensional, color Doppler, Doppler ultrasound characteristics, in accordance with the extent of lesion classification, and detect concentration its peripheral blood plasma endothelin (ET) and serum nitric oxide (NO) ,analyzing relationship of the degree of concentration of both change and the relevance of ultrasound. I. Materials and Methods:This study chooses 90 cases of untreated patients with arteriosclerosis obliterans to have color Doppler ultrasonography examination, and use two-dimensional color Doppler ultrasound to observe the vascular diameter, in-diameter, wall thickness , smooth wall, calcium plaque distribution, a narrow percentage of segmental occlusion, and collateral formation, such as color hemodynamic changes and so on. Observed in 1995 reference clinical stages of arteriosclerosis obliterans standards and the degree of arterial stenosis criteria, in order to make this Group study color Doppler ultrasound classification:Mild: Arterial segments wholly or in part to severe stenosis, without occlusion; segmental occlusion or not more than one, and surrounded by color Doppler shows that collateral circulation has been established, other arterial lumen of light to moderate stenosis.Moderate: arterial occlusive segment 2 ~ 3, color blocking blood flow has been around 2 to 3 artery establish collateral circulation.Severe: arterial occlusive segment 4 above or artery occlusion whole, without collateral circulation, or the establishment of the following two, other arterial lumen to severe stenosis.Collecting peripheral blood of patients, measuring concentrations of the plasma endothelin (ET) and serum nitric oxide (NO) , while collecting 20 cases as a normal control group. Finally, have the test results statistics analyzed. â…¡.Results:This group of patients with arterial occlusion: common iliac artery (CIA) 11 (12.2%), external iliac artery (EIA)12 (13.3%), common femoral artery (CFA) 16 (17.8%), femoral artery 51 (FA)(56.7%), popliteal artery (POA) 31 (34.4%), anterior tibial artery (ATA)46 (51.1%), posterior tibial artery (PTA) 30 (33.3%), dorsalis pedis artery (DPA)21 (23.3%).Other artery showed varying degrees artery stenosis.At the same time, this group showed that a total of 102 collateral arteries, a majority of the collateral artery vascular access, the more visible distal arterial blood flow signal. The establishment of collateral circulation plays a role that can not be overlooked to the distal limb occlusive arterial blood supply.90 patients with arteriosclerosis obliterans (ASO) patients had high levels of low NO ET hyperlipidemia and hypertension, ET in ASO group was significantly higher than in normal control group (P <0.01); serum levels of NO was significantly lower than the control group (P < 0.01), patients with high ASO ET hyperinsulinemia and low NO hyperlipidemia, with the ASO lesions and color Doppler ultrasonography showed that the extent of arteriosclerosis obliterans segment and increased gradually worsened. Severe Group ET level was significantly higher than that mild, moderate group (P <0.01); NO content was significantly lower than mild, moderate group (P <0.01). And the ET and NO moderate level compared with the mild group, also has highly significant difference (P <0.01). â…¢.Conclusions:1. Extent of atherosclerosis and blood ET, NO relevance of the degree of changeThe results showed that serum NO group, the abnormal plasma ET with the ASO, development and disease closely related. ASO group serum levels of NO were significantly decreased than normal group , and with arteriosclerosis obliterans segment increased the extent of increase in reduced, the plasma levels of ET increased significantly than normal control group, the two were significantly related to the severity, such as a wide range, a high level of plasma ET.2. Color Doppler ultrasound could classify ASO on the whole extent of the lesionThis is because that the current grading standards are on the arterial partial arterial segment stenosis of observation to determine, not analysis of the overall changes in the lower extremity arteries, and clinical staging of patients with clinical manifestations mainly from the clinical manifest, not really see the changes in the actual extent of artery stenosis and occlusion of the segments. And because ASO group patients were elderly with poor capacity of perception, often complained of symptoms is not consistent with the actual artery change extent. Therefore, this study classified the lower extremities arteriosclerosis obliterans grade rating by color Doppler ultrasonography in the whole of the patients has positive significance. 3. This study proves that the blood ET, NO changes and ASO illness degree is closely related from the Imaging perspective Through examing the ASO patients with color Doppler sonography and detecting blood ET, NO, confirmed that blood ET, NO changes of ASO and the artery lesions are closely linked from the image field of science. The group revealed that the level of plasma ET in ASO group was significantly increased compared with the normal control group, and was proportional with the severity of the disease. The level of NO in ASO severe group was significantly lower than mild, moderate, moderate NO group was significantly lower than mildly; the level of ET in ASO severe Group was significantly higher than that mild, moderate group, moderate group was significantly higher than that of mild Group. Analysing the variance for each group of indicators, all the results showed that P <0.01, with statistical significance.4. The results of blood ET, NO in this study provided a classification based on laboratory for the new classification method by color Doppler ultrasoundColor Doppler ultrasound can observe and evaluate the extent of arteriosclerosis obliterans intuitive and accurately, establish the classification of the ASO on the basis of lower extremity a extent of arterial changes in the overall level from the macro. And through ultrasound classification, estimating the relationship between ET,NO and the severity of arteriosclerosis obliterans. And it could provide early, sensitive and accurate diagnosis and technical indicators for clinical,and has great significance in the early diagnosis, prevention and the treatment of arteriosclerosis obliterans. At the same time, in this study, the classification by color Doppler ultrasound has objectivity, accuracy and feasibility, the NO, ET diversification of concentration in three groups of patients has provided strong laboratory evidence.5. The establishment of collateral circulation of ASO is vail to the distal arterial blood supplyThis study revealed that the distal arterial blood supply could be improved when the ASO patients has better collateral circulation. Especially when the degree of the arterial lesions closed to the heart is heavy, but there is good collateral circulation, the distal arterial blood flow signals often filling good. Therefore, the establishment of collateral arteries in patients is of positive significance to improve the blood supply situation of distal arteries.Color Doppler ultrasound could detect the issue and access of collateral arteries accurately and sensitively, and can show collateral and distal arterial blood flow clearly, and it is a reliable method to evaluation the disease extent of arteriosclerosis obliterans.
