Objective: To explore the value of postoperative prophylactic portal vein chemotherapy (PVC) on delaying recurrence in patients with Hepatocelluar carcinoma (HCC) of Stageâ…¡and to evaluate risk factors of recurrence in this study .Methods: 62 patients with clinical stage II HCC pathological proved who underwent radical resection from Feb. 2002 to Feb. 2007 in Xinjiang Tumor Hospital, were reviewed.The patients were followed up till November 2007. 20 patients underwent radical resection only and 42 patients underwent prophylactic PVC after operation. Disease-free Survival statuses and median survival time of the patients were analyzed by Kaplan-Meier method; Clinical and pathological factors such as preoperative (alpha-fetoprotein)AFPlevel, number of nodules, tumor thrombus, cirrhosis, prophylactic PVC, were analyzed by COX model. Results: The 0.5,1 and 2 -year disease-free survival rates were 45%,34.3%,17% in operation only group, and 74.7%,57.5%,39.9%in operation combined with PVC group. The median survival time was significantly higher in operation plus PVC group than in operation only group (23.1 months vs. 5.6 months, P<0.05). Cox multivariate analysis showed that : number of nodules ,tumor thrombus,monocytes counts and prophylactic portal vein chemotherapy were the factors of the recurrence for stage II HCC patients( P<0.05). Furthermore prophylactic portal vein chemotherapy significantly increases the 1 year disease-free survival rate after operation. Conclusions: Prophylactic chemotherapy can delay early recurrence of the stage II patients with risk recurrence factors for hepatocellular carcinoma, which is meaningful for only one year's results.
