Objective: To evaluate the value of diffusion-weihghted MR imaging's apparent diffusion coefficient(ADCs) in the diagnosis of the maligant bone neoplasms. Methods: Nine cases with consective primary malignant bone neoplasms of the lower limbs were prospectively enrilled in the study.Each patient had a controlled preoperative MRI including a SSEPI-DWI squence. With the help of a pothologist,spatially localized histological samling study of the postoperative specimens was performed.ADCs of vible tumor, peritumoral edema,and tumor necrosis of the neoplasms confimed by spatially localized histological sampling or following up were calculated and compared. Results: Mean ADCs of normal muscle was (0.433±0.136)×10-3mm2/s ;mean ADCs of normal marrow was (0.471±0.114)×10-3mm2/s;mean ADCs of muscle edema was (2.326±0.144)×10-3mm2/s;mean ADCs of marrow edema was (1.848±0.176)×10-3mm2/s; mean ADCs of tumor nerosis was (2.018±0.174)×10-3mm2/s;mean ADCs of viable tumor was (1.379±0.165)×10-3mm2/s。Statistical differences were significant among all these tissues (F=246.761,P=0.000).The ADCs of viable tumor were higher than that of normal marrow(p<0.05), normal muscle(p<0.05), and lower than muscle edema(p<0.05),marrow edema (p<0.05) or tumor necrosis(p<0.05).Conclusions: ADCs calculation will be a quantative method in differentating viable tumor from peritumoral edama, and viable tumor necrosis..
