Objective: to investigate the etiology, clinical and radiological features, prognosis of the perimesencephalic nonaneurysmal subarachnoid hemorrhage (PNSH). Method: retrospectively analysis the clinical presentation, radiological materials, treatment and prognosis of 37 perimesencephalic nonaneurysmal subarachnoid hemorrhage (PNSH) cases who were consecutively treated in our neurosurgical department, meanwhile we carry out a critical review of the literature of PNSH. Results: all cases were conscious, whose Hunt and Hess scale were Gradeâ… orâ…¡.all cases had a headache, 35 cases had vomiting, 36 cases had positive meningismus, 4 cases had a slightly focal neurological deficient. All cases were fully recovered when they discharged. The follow-up ranging from 4 to 72 months, mean 40 months. There was not rebleeding, cerebrovascular vasospasm and hydrocephalus. The quality of life in the long term was excellent after symptomatic care. Conclusion: patients with PNSH have an even clinical condition, unique radiological features, short-term convalescence, less complication and excellent prognosis. Rightly recognizing and diagnosing PNSH can reduce the in-hospital time, repetition of DSA and unnecessary surgical exploration.
