Objective: To investigate the correlation between DWI exhibition, ADCvalue and pathologic features of urine pathway epithelium neoplasm.Materials and Methods: 37 cases of urine pathway epithelium neoplasm and9 cases of control benign lesions were respectively scanned with MRI andDWI and examined by tissue pathologic method (HE staining), and then thecorrelation of DWI exhibition and ADC value with related pathology changeswas analyzed. Results: 37 cases of urine pathway epithelium neoplasmwere showed significant hyper-intensity signal in DWI images that didnot decrease with increasing b value. 15 cases of infiltrating urothelialcarcinomas were invaded muscle layer and ectoptygma and 22 cases ofnon-invasive urothelial carcinomas without muscle layer infiltrating werelimited within the layer of mucous membrane. The diagnosis accuracyrate of DWI was 94.6%(35/37). ADC values were ( 1.013±0.089)×10-3mm2/s and (1.020±0.105)×10-3mm2/s in high and low gradeinfiltrating urothelial carcinoma respectively and were (1.224±0.097)×10-3mm2/s and (1.248±0.113 )×10-3mm2/s in high and low gradenon-invasive urothelial carcinoma respectively. There was no remarkabledifference of ADC values between the high-grade and the low-grade ofinfiltrating urothelial carcinomas and between the high-grade and thelow-grade of non-invasive urothelial carcinomas. However the ADC values were significant different between non-invasive urothelialcarcinomas and infiltrating urothelial carcinomas(P<0.05); 12 cases of 15infiltrating urothelial carcinomas cases with lymph node metastasisshowed high signal in DWI image that did not decrease with increasing bvalue, similar to the 9 benign enlarged responsive proliferative lymphnodes. But the former showed higher signal than the later. The ADCvalues were (1.119±0.138)×10-3mm2/s and (1.724±0.1115 )×10-3mm2/srespectively, and showed significant different between two groups(P<0.05). Conclusion DWI and ADC values are secure, scatheless,accurate, and quickly methods to diagnose urine pathway epitheliumneoplasm and can exactly judge invasive degree and lymph node situation thatcan provide exactly TNM evaluation.
