Objective: To explore the situation of cervical dysplasia and carcinoma in dongguan,to evaluate the accuracy and reliability of DNA image cytometer in early screening of cervical dysplasia and carcinoma and put forward the reasonable project of screening of cervical dysplasia and carcinoma.Methods: A total of 6000 women of child-bearing age in dongguan randomly selected by chester sampling were screened by DNA image cytometer,each doubtful cases was followed by colposcopic examination where biopsies were taken. 200 healthy women compared with 50 cervical carcinoma patients in the people's hospital of Dongguan were examined by DNA image cytometer to obtain the sensitivity,specificity,false negative rate and false positive rate of this method.Results: a total of 6000 women were screened for cervical carcinoma. The age of the examinees was between 19 to 84,the average was 34.68. The distribution of ages were listed respectively as following: 962(under 30 ages), 2499(ages 31-40),1890(ages 41-50),578(ages 51-60),70(ages 61 and up). There were 219 DI>2.5 cases in 6000 samples (3.65%). The pathological diagnosis was cervical dysplasia and carcinoma in 51 cases (0.85%). In the control group of 50 cervical carcinoma patients and 200 healthy women,the number of true positive cases is 45,false positive is 31,true negative is 169,false negative is 5. So the sensitivity of the DNA image cytometer is 90.0%(45/50),specificity is 84.5%(169/200),positive predictive value is 59.2%,negative predictive value is 97.1%,Youden Index is 75.5%. All of that is higher than the conventional cytology. Conclusions: The screening for cervical dysplasia and carcinoma by DNA image cytometer could be higher sensitivity,enhanced accuracy and better repeatability. And it is worth being generalized in large-scale screening to instruct the project of early therapy and improve prognosis.
