This paper presents a three-dimensional(3D) pre-surgical planning and image-guided surgical system for neurosurgery has been proposed. Firstly, the system can reconstruct and display a patient brain structure in 3D model. Then the system allows surgeons to target the region of interest (ROI), plan the corrective surgery path, and simulate both the preoperative structure and the predicted postoperative state in 3D. After the surgeons finished the surgical planning, fiducial-based registration method is applied to the real patient coordinate and the 3D brain model coordinate. Furthermore, the alarm function during the operation ensures the surgeon to accurately follow the planned route, and accurately implement the procedure. Experiment results of a phantom head reveal that the error rate of the system is less than1.2 0.5mm. It indicates that proposed navigation system can significantly improve the accuracy of the neurosurgical procedure.To testify the proposed system, a PVA head phantom model (designed by Asia-Pacific computer Co., Shanghai, China) is employed and it was scanned by a SIEMENS SOMATOM Sensation 16 CT machine. The data is in dimensions of 512X512X200, with pixel distance of 0.5859mm and slice distance of 1mm. As the result, the average broad sense registration error (contains the scan error of CT, the error of NDI infrared-track device and registration error) of the system is 1.2±0.5mm, which is significantly improved from existing system in China, and suitable for a real clinical application. We are working on more real clinical testing and accuracy enhancement.My contributions in the system are as follows:The first one is the Oblique panel, with the help of the oblique panel; our surgery could easily catch the issue around the planning path.The second is the Play function, after our surgery set the planning path, the play function could show the issue immediately, it is a great help to our surgery.The third is the Guidance function, it could immediately alarm our surgery that if the real path is not along the planning path.
