Objective: To evaluate the effectiveness of systemic hepatectomy and the correlation factors of complication for the patients with primary hepatic carcinoma(PHC)Methods: Analyze the clinic date of 50 patients diaglosed with PHC in our hospital between June 2005 and November 2007.Record disposition of the tumor on the liver, hours spend on operation, blood loss during operation, and the postoperative complication. Compare the level of serum albumin with postoperative complication ussd chi-square test and t-test.Results: In the 50 cases, there were 15 cases on pro-right of liver, 19 cases on retro-right of liver, 5 cases on intra- left of live, 7 cases on extra-left of live, 2 cases on lobus quadratus hepatic and 1 cases on caudate lobe. All these operations were completed successfully. Time spent on the operation was ranged from 2.0 to 6.0 hours, 206 min in average. Bood loss during operation was ranged from 200 to 6000 ml, 1800 ml in average. According to the first serum hepatic function analysis, the level of seralbumin was ranged from 23.1g/L to 48.5g/L. There were some complication happened, such as postoperative bleeding, pleural fluid, abdominal dropsy, liver function failure, infection of incisional wound etc.. To compare, there were significant differences between the different levels of seralbumin and the morbility of complications(P<0.01)Conclusion: The systemic hepatectomy could achieve better therapeutic effect with less complication, less blood loss, less possibility of recidivation. It is an effective and chiefly way in the treatment of PHC, and It is also an important way to ensure the safety of hepatectomy.
